  • Salary: $27.59/hour
  • Schedule: 9AM-5PM, Monday- Friday
  • Location: 320 Carleton Ave. in Central Islip NY

 

What drives the core purpose of this job:

The Case Manager Senior is responsible for all aspects of Home-based Crisis Intervention and case management services for client families in the Alternatives for Youth Program.

 

What are the key Responsibilities:

  1. Assist families in resolving the crises and problems that led to the parent(s) seeking a PINS petition through case management and by providing support to families in crisis struggling with problems such as substance abuse and mental illness.
  2. Provide intensive, home-based case management and crisis intervention to help the family reach stability.
  3. Conduct assessments using the Child and Adult Needs Assessment (CANS) instrument and develop a Family Intervention Plan based on family needs, using a strengths-based approach.
  4. Make referrals to partnership agencies and outreach to new agencies when required.
  5. Follow-up with service providers to monitor families progress.
  6. Assist parents and youth in learning behavior management techniques.
  7. Maintain progress notes and accurate case records into the computer system with all necessary data entry in accordance with contract mandates.
  8. Be on call to families contacting the AFY hotline as scheduled.
  9. Transport and escort clients from appointments or to obtain emergency resources such as clothing or medication as needed in own vehicle when necessary.
  10. Participate in team meetings, case conferences, agency presentations, and trainings.
  11. Serve as a resource to case managers in specialty areas of mental health and/or substance abuse.
  12. Assist with the training of new staff.
  13. Supervise interns and volunteers.
  14. Assign cases received by the Department of Probation to Case Managers in the Program Directors absence.
  15. Oversee the maintenance of agency vehicles.
  16. Facilitate and develop monthly educational workshops for adolescents when needed.
  17. Participate in monthly recreational events for the adolescents in the AFY program when needed.
  18. Customarily and regularly exercises discretionary powers
  19. Hold weekly female and male adolescent groups when assigned by the Director.

What key knowledge, skills, and abilities are required to excel in this role:

  1. Masters Degree and SIFI certification required with at least 3 years of experience in the area of mental health or substance abuse treatment.
  2. Participation in at least 20 hours of training in the first six months of employment (with an additional 20 hours during the second six months of employment) on Homebuilders, mediation, working with adolescents, substance abuse, mental health, the juvenile and/or criminal justice system, and related topics. Training will be provided by outside trainers, EAC NETWORK and by partnering County agencies.
  3. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
  4. Must have good communication skills, both oral and written.
  5. Must have valid New York Drivers License and access to an automobile.
