Full-Time Primary Care NP/PA
Job Description
Transhealth is a new health center dedicated to providing comprehensive gender-affirming clinical care, research, advocacy, and education in partnership with our community. We are seeking a full-time Nurse Practitioner or Physician Assistant to provide exceptional adult primary care to gender-diverse patients.

Responsibilities
Clinical Duties (70%)
- Provide ongoing primary care of patients
- Utilize clinical practice knowledge to identify, prevent, or solve complex and/or recurring patient care problems
- Follow and manage patients to enhance continuity of care
- Collaborate with clinical staff to facilitate a comprehensive plan of care and coordinate resources
- Monitor patient outcomes in accordance with defined standards of patient care
- Provide chronic disease management
- Participate in integrated behavioral health treatment interventions as needed
- Respond to medical emergencies as appropriate
- Follow patients admitted to local hospitals as needed with care coordination after discharge planning
- Work closely with the ambulatory nurse to ensure appropriate care coordination services are provided
- Participate in rotating evening & weekend on-call coverage
- Other duties as assigned
Administrative Duties (20%)
- Serve as a resource to staff in area(s) of expertise
- Complete medical record documentation, billing, prior authorization/ referral documentation in a timely fashion
- Collaborate with managers, clinical nurses, and other clinical staff to plan, implement, and evaluate the pertinent changes in practice and health care delivery
- Identify system-wide factors contributing to undesirable outcomes, such as complications, morbidity, mortality, and overutilization of services
- Participate in ongoing quality assurance programs
- Attend staff meetings
- Participate in population management activities
- Evaluate own practice and program effectiveness
- Other duties as assigned
Community Duties (10%)
- Develop community-based initiatives aligned with the mission, vision and values of the organization
- Participate in various community engagement activities with our various community partners
- Participate in advocacy to advance patient-centered gender-affirming care
- Participate in the development of formal and informal educational programs for the staff
- Conduct independent research pertaining to the needs of the communities we serve
- Evaluate current research findings for clinical specialty and determine applicability to our community
Skill/Education Requirements
- Experience working with the LGBTQ+, and specifically, the transgender community is required
- Current licensure in the Commonwealth of Massachusetts as a registered NP or PA
- Master’s Degree in Nursing or completion of NCCPA program
- Current certification as an NP or PA in an appropriate specialty to care for adult patients
- 2 years of advanced practice experience preferred
- Clinical knowledge and skills based on current health care practice/ research
- Willingness to learn and use Slack, G Suite, and other programs

Reporting Structure
- This position will report to the Primary Care Director
Job Type: Full-time
Pay: $100,000.00 – $150,000.00 per year

- 8 hour shift
- Day shift
