Job Description
Title: Social Services Coordinator
Department: Health and Human Services
Supervisor: Health Director
Status: Full-time (35 hrs/wk), Local 272 Clerical, Level 13, comprehensive benefits & PTO.
Starting wage $31.18 to 34.58 per hour
The Social Worker will provide the necessary services to help community members facing housing insecurity, food insecurity, and other emergent cases. In many situations, Social Workers interact with children and family members, including siblings, parents, extended relatives, and guardians in order to assess the needs of individuals and families to determine the best course of action for
improving the living environment.
Duties and Responsibilities (these duties are a general summary and not all inclusive):
– Respond to complaints in a professional and timely manner following standard operating procedures; provide customer service with tact, courtesy, sensitivity and discretion in all dealings with internal and external customers.
– Processes referrals from private individuals, social service agencies, community departments, and faith organizations.
– Assesses referrals and determines a case management plan.
– Facilitates the case management plan by offering any available resources.
Coordinates the delivery of services through advocacy and assistance with applications for benefits.
– Responds to crisis incidents in collaboration with the Health and Human Services Department, Library, Council on Aging, School Department, Food Pantry, Housing Authority and Public Safety Departments.
– Maintains a database of available services for public access.
– Maintains records (phone logs, electronic confidential client files) and develops system for documentation and monthly statistic reports.
– Complies with mandated reported guidelines and reports information to the appropriate authorities.
-Attends events providing community education and seeking outreach, hosts community workshops, support/focus groups, and seminars.
-Assists in the development of programs according to the initiatives of the strategic plan of the Health and Human Services Department, in collaboration with all Community Service Departments.
Assists as needed with emergencies within the City (fires, flooding, gas leaks, health emergencies, etc.).
Respond sensitively and constructively to citizens; provide customer service with tact, courtesy, cultural sensitivity and discretion in all dealings with the public
Contribute to creating a collaborative work culture that appreciates diverse perspective and approaches matters with flexibility and cultural relevance.
Qualifications
-Bachelor’s degree in Social Work, or Bachelor’s degree in related field and LSW license, or equivalent experience and license. Experience working with families, elderly individuals, and children is strongly preferred.
-Excellent communications and interpersonal skills; demonstrated experience providing customer service to the public using tact, compassion, proper judgment, courtesy, strictest respect and maintenance of confidentiality.
-Proficiency using MS Office Suite (Excel, WORD and Outlook) with a demonstrated ability to learn new software programs.
-Experience along a full age spectrum; geriatric experience and addiction knowledge
-Ability to find solutions, use discretion and cooperate with customers and ability to de-escalate and re-cue consumers while practicing good boundaries.
-Valid and current MA Driver’s License and a driving record which proves responsible and lawful driving habits.
-Demonstrated ability to maintain positive public relations and work effectively with diverse constituencies ensuring a culturally relevant and sensitive approach.
-Ability to successfully pass a background check.
Physical Demands:
While performing this job, the employee must be able to drive a vehicle, climb stairs and possibly enter confined spaces and is required to sit, stand, walk, reach, climb, stoop, crouch and twist. The employee is required to hear, feel and talk. The employee is required to lift heavy objects up to 30 pounds and carry these objects short distances. The employee must have close, distance, and peripheral vision, depth perception and the ability to adjust focus. The employee must have the
ability to work safely under difficult work situations, during inclement weather conditions including during rain, snow, wind and ice storms and freezing temperatures.
