Job Summary
The Alliance Health Project (AHP) is a community-based LGBTQ+ and HIV-focused clinic located at 1930 Market Street in San Francisco. We provide psychotherapy, substance-use counseling, case management, support groups, psychiatry, sexual health services, clinical traineeship, and provider education and training. Our multi-disciplinary staff are a client-centered, collaborative, Harm Reduction-oriented team. We have continued to grow as a community-based clinic by anticipating and addressing the changing needs of our LGBTQ+ and HIV community members for more than 35 years. Our process of continuous improvement of service to our community includes a strong focus on cultural humility and anti-racist service provision.

The Intensive Case Manager position is part of the Behavioral Health Services team at the 1930 Market Street Service Center and is responsible for providing case management services to individuals and groups who have substance use disorders, potential co-occurring mental health disorders and/or are living with HIV/AIDS. This position is part of a larger multidisciplinary behavioral health services team that includes psychiatrists, nurses, social workers, therapists, substance use counselors, case managers and psychologists.

This position provides intensive case management services to individuals living with co-occurring diagnoses of substance use issues and severe mental illness, within a Harm Reduction philosophy. Motivational Interviewing, among other interventions, is utilized to help clients explore their relationship to their drug(s) of choice, address their ambivalent feelings about making changes to their use and creating client-centered, realistic and meaningful treatment plan goals for change. This position works under the clinical supervision of a licensed clinician and in conjunction with other teammates. This position will involve providing case management services consisting mainly of field work, linkage to HIV medical care, assisting with retention in care, assessing clients’ service needs and assisting clients with appropriate referrals, including for housing, vocational and other mental health services. Additionally, the Intensive Case Manager will assist clients in creating and meeting treatment plan goals and reduce barriers to care. This position will involve co-location and working closely with Zuckerberg San Francisco General Hospital Ward 86 HIV clinic POP-UP (Positive-health Onsite Program for Unstably-housed Populations) Team, providing case management services to chronically unhoused clients who routinely fall out of medical care and engagement in medical treatment, within a multidisciplinary team.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% – 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Required Qualifications
• CSW I: Master’s degree in Social Work, Marriage and Family Therapy or related Counseling field from an accredited school of Social Work, Counseling or similar and knowledge and abilities essential to the successful performance of the duties assigned to the position or equivalent combination of experience/training
• CSW II: In addition to the above, possession of the requirements necessary for application for licensure as a LCSW/LMFT/LPCC as determined by the California Board of Behavioral Science Examiners.
• A preliminary job offer and assignment may be extended pending successful background clearance.
• Within 12 months of employment, successful completion of SF Department of Public Health Involuntary Hold/5150 certification.
Preferred Qualifications
• One year of experience providing case management work
• Experience working with the HIV-positive and LGBTQ+ communities
• Bilingual in English and Spanish – written, reading, and oral proficiency.
• Bicultural, Experience working collaboratively with Spanish-speaking service providers
• Familiar with Avatar, Epic, ARIES and Substance Use and Mental Health systems of care,
• Understanding of psychosocial issues facing the LGBTQ and HIV-affected communities.
• Experience working within Harm Reduction treatment models, utilizing Motivational Interviewing interventions
• Familiarity with evidence-based substance use treatment modalities
• Sensitivity to working with cultural, ethnic, and racial issues.
• Understanding and experience working with severe mental illness.
• Excellent communication and interpersonal skills.
• Demonstrated excellence in reliability and attendance.
