Expected Starting Salary: 22.79 – 27.77

PURPOSE OF POSITION: To assist in the integration and providing of services by all members of the health care team pertaining to unit clients. Role model and promote collaboration and communication among members of the healthcare team. Manage the milieu on the unit. Work towards optimal clinical and resource outcomes with timely discharges. To monitor, analyze, and report on resource utilization during the providing of patient care.

Responsibilities

  • Compliance – Patient Services Staff
    Sustain a working understanding of regulatory requirements and accreditation standards. Support leadership in Compliance efforts to meet and sustain the regulatory compliance efforts of the department and hospital. Be mindful and judicious in monitoring safety and departmental policies and procedures. Ensure all individual certifications and Safety College requirements are fulfilled in a timely basis. Work with department managers, nurse council and magnet representatives to identify and share safety best practices. Be mindful of CCHMCs processes and compliance with CMS standards and freely communicate safety and compliance concerns to leadership team members.
  • Interprofessional Practice:
    Demonstrates consistent integration of the Interprofessional Practice Model (IPM) in all aspects of practice.
  • Patient Care – Age Specific and Culturally Diverse
    Consistently integrates age specific and culturally diverse concepts into patient care, taking into consideration both the patient’s chronological age and developmental functioning.
  • Patient Care
    Coordinates care of patents through appropriate patient placement and care provider assignments. Effectively communicates clinical information concerning patients. Collaborates with other healthcare providers promoting smooth transitioning to different levels of care. Monitors staffing levels and acuity needs. Facilitates patient processing by collaborating with family, interdisciplinary team members and discharge support staff. Facilitates procurement of services and resources.
  • Leadership
    Promotes Bullseye philosophy, role modeling and takes opportunity to teach others by example of own work performance. Utilizes critical think skills reflecting global thinking, by use of sound judgment, problem solving processes and conflict management to effectively develop ideas and concepts.
  • Planning
    Facilitates the involvement of patients/families and providers in care decisions, goal setting and discharge planning. Assists with the coordination of multi-departmental resources and provides support to healthcare providers as it relates to the discharge process.
  • Resource
    Serves as a resource for interventions to improve clinical outcomes. Participates in improvement activities and provides input in department discussions focused on improving patient care.

Required:

  • Bachelor’s Degree in a related field (Social Work, Counseling, Psychology, Special Education, etc.
  • 2 years related experience working directly with behaviorally complex patients.Most notably working with children who may have the following: Autism Spectrum Disorder, Developmental Delay, Oppositional Defiant Disorder, Psychosis.

Preferred:

-Person who can adapt to complex situations and respond effectively in crisis situation.

-Person who has the ability to learn about medical complexities and have the ability to collaborate with medical teams by discussing patient behavior and subsequent interventions utilized for patient care.

-Person who can provide direct behavioral support to the behavioral needs of patients.

-Person who can communicate behavioral concepts clearly to others who may not have behavioral knowledge.

  • Master’s Degree in a related field
