Position Summary
The APRN provides comprehensive health care for the Geriatrics specialty This includes completion of a health assessment, determining diagnosis, planning and prescribing treatment. The APRN functions as part of the multidisciplinary health team in accordance with privileges approved by the credentialing committee to provide high-quality, cost-effective advanced care to patients. The APRN participates in the educational experiences of nursing students, medical students, residents and other health care disciplines. The APRN in Geriatrics will focus on the care of older adults, including but not limited to preventing and treating diseases associated with aging, managing multiple chronic conditions, screening for and managing geriatric syndromes. Clinical duties will include a combination of outpatient care, inpatient consultations, and providing care in skilled nursing facilities. This position requires provision of care at more than one location.
Duties & Responsibilities
• Works collaboratively with other providers in the care of patients. • Obtain relevant patient health history. Perform physical exam and record findings. • Orders and interprets tests, treatments, and therapies within the scope of practice. • Perform various diagnostic, preventive and/or therapeutic procedures and/or order special screenings and developmental or laboratory tests to diagnose conditions; interpret results. • Prescribe/order appropriate pharmacologic and non-pharmacologic interventions and develop a treatment plan. • Provides prevention strategies and management of acute and chronic health problems. • Order appropriate consultation/referral. • Performs procedures according to clinical delineation of privileges. • Develop an educational plan for patient/family. • Prescribes medications, supplies, and durable medical equipment, according to clinical privileges. • May provide other clinical duties within the scope of practice as directed under the emergency operation plan. • Integrates evidence-based knowledge and clinical expertise in decision-making. • Participates in the educational experiences of nursing students, medical students, residents and other healthcare disciplines. By being involved in case conferences, serving as a preceptor and mentor, assisting with curriculum development and instruction. • Demonstrates fiscal responsibility to positively impact the financial performance of the organization while ensuring optimal patient care and safety. • May lead and assist in research and evidence-based practice through participant selection, coordination of the protocol(s), data collection, and other associated activities. • Demonstrates accountability for professional conduct. • Advances professionalism by seeking opportunities to develop new skills and to enhance current ones. • Provides formal and/or informal education or mentorship to both internal and external groups pertinent to area of specialty. • Participates in program development. • Acts as a resource for clinical staff in the practice setting. • Consults with management in assessment, problem-solving, decision-making, and evaluating clinical/system issues and models of care. • Identifies and participates in quality-improvement initiatives within the practice setting and initiates change as appropriate. • May participate in the development and revision of protocols or guidelines to reflect evidence-based changes in care management. May participate in activities that influence policy-making bodies to improve patient care. • The Advanced Practice Registered Nurse is required to have and maintain a faculty appointment in order to receive clinical privileges to practice at the University of Illinois Health System. • Perform other related duties and participate in special projects as assigned.
Minimum Qualifications
- Master’s Degree in Nursing or Doctorate of Nursing Practice required.
- Graduation from accredited APRN program.
- Current license as a Professional Registered Nurse and Advanced Practice.
- Registered Nurse in the State of Illinois required.
- Current American Heart Association Basic Life Support (BLS) Certification required.
- Current Controlled Substance licensure and Federal DEA required.
- Effective communication skills (oral and written) that facilitate therapeutic relationships between physicians, APRN and PA providers as well as multi-disciplinary care team.
Preferred Qualifications
- Experience working in primary care, geriatric care, and/or inpatient settings as an APRN
