Full-Time Community Projects Coordinator
Job Description
The Community Impact Team (CIT) with the Health District is seeking an individual with experience in improving health systems and addressing health-related social needs who is preferably bilingual/bicultural and/or brings a unique perspective to the role of Community Project Coordinator. The ideal candidate is an innovative thinker, relationship builder, problem solver and strategic planner, who is knowledgeable about health-related social needs, social determinants of health, and passionate about advancing equity and improvement of health systems-level change. The position reports to the Community Impact Team (CIT) Manager and works closely with community partners, Health District leaders and others to gather, organize, and utilize information leading to the organization and implementation of improving health system service delivery and addressing health-related social needs.
Key Responsibilities
- Use research, data, information and community input and analysis to help the community develop and implement short and long-range solutions to community health-related social needs and health service delivery.
- Work closely with internal leaders, community partners and stakeholders.
- Develop effective materials and presentations.
- Plan and organize meetings and events with partners and stakeholders.
Pay Range and Status
- $62,720 to $76,832 per year, dependent on qualifications (DOQ)
- 40 hrs/wk

Education and Experience
Any combination of education, training, or experience that would provide the equivalent of:
- A minimum of a bachelor’s degree in social work, public health, health administration, or other relevant field, OR a combination of equivalent experience and education.
- A minimum of five years of experience, demonstrated ability and skills in a health, public health, human services, or other relevant setting or field.
- Project management and meeting deadlines.
- Implementing and evaluating programs and services.
- Creating and delivering impactful presentations.
- Problem-solving and collaborating for long-term improvements.
