Full-Time HCV Program Coordinator

Job Description

Purpose: “Working for and within populations adversely affected by stigma, poor access to care, discrimination, and poverty, AIDS Care Group is dedicated to using its resources to provide comprehensive and compassionate clinical care and social services to persons living with leading public health conditions including, but not limited to, sexually transmitted diseases, HIV/AIDS, hepatitis C, mental illness, and opioid use disorders.”
It is the responsibility of the HCV Program Coordinator to provide clinic-based, off-site and other community level support to HIV/HCV positive clients. The first focus of this is always medical, but housing, provisions of daily living, social and survival (clothing, food, etc.) needs, are additional responsibilities.

Responsibilities:
• Greet and acknowledge all patients according to agency standards
• Receive, record and relay messages accurately, completely and legibly
• Speak with others using clear, professional and polite language
• Develop and maintain positive working relationships with others, support the ACG
team to reach common goal
• Communicate with patients, employees, and other individuals to answer questions, disseminate or explain information, take orders, and address complaints
• Intake and assessment of new HIV/HCV positive patients
• Review benefit/insurance status and enable this coverage to better facilitate AIDS Care Group treatment and to better facilitate their pharmaceutical coverage
• Obtain referrals for patients who are obtaining MONO HCV care at the AIDS Care Group
• Link these clients to all provisions of care
• Act as the main communicator and coordinator for assigned drug and treatment centers as well as other sites in the collar counties (Berks, Bucks, Chester, Delaware and Montgomery) around Philadelphia. This to be done under close association with the Division Director
• Monitor and help navigate all Co-infected and MONO HCV patients through the process of completing HCV treatment
• Maintain and update HCV & HIV testing data bases (outside of current STD related testing data).
• Build all physical HCV & HIV testing folders as needed.
• Assist the HCV Supervisor in the management and movement of HCV medications, ensuring that all refills are completed, and all medications are obtained when needed from several different pharmacies including Elwyn, Silver Scripts, and Bravio.
• Serve as back-up for the HCV Supervisor in Compiling and completing all HCV applications, and present to the respective providers
• Be responsible for answering and problem solving all denials from insurance companies which is completed as letters and phone calls.
• Review HCV files and provide support to medical staff in completing paperwork for these files
• Work evening hours as assigned and occasional Saturdays to coordinate with agency efforts on weekends. Demonstrate the ability to travel to various satellite locations and to best address these client needs
• Learn HIV/HCV testing methods and provide these rapid and other tests on clients and in the agency clinic locations
• Regularly attend meetings, trainings and especially correctional trainings, meetings and event to progress the goals of this special treatment initiative. Attend conferences and report findings to staff per trainings at agency events
• Maintain company vehicle for transportation of clients as needed
• Other duties as assigned.

Requirements/ Qualifications:
• High School Diploma (or GED equivalent with skills in general office practice).
• Experience working in a medical office setting preferred
• A working knowledge of the community and the ability to link clients to services in the community when necessary
• Valid Driver’s License
• Must be able to comfortably work with people of all lifestyles

• Experience with electronic health record system

Work Conditions:
• Frequently required to stand, walk and sit for longer periods of time.
• Exposed to hazards of office such as files cabinet drawers, desk drawers, boxes being delivered that may need to be moved and or picked up
• Subject to paper cuts
• Occasional stooping, reaching, bending, kneeling, crouching.
• Occasional lifting of equipment or supplies (up to 50lbs).
• Handle emergency or crisis situations.
• While performing the duties of this job, the employee is regularly required to talk and hear
• May be exposed to chemicals
• May be exposed to diseases and infections
