ESSENTIAL DUTIES & RESPONSIBILITIES
- Provide orientation to the facility to new clients
- Oversee room maintenance and enforce housekeeping hygiene guidelines in a trauma-informed manner
- Manage the clean linen exchanges once a week
- In occasion provide one-on-one personal care assistance to residents as directed (grooming, feeding, and other support).
- On occasion, help transfer, lift, raise, and turn residents; utilize proper lifting and body mechanics to prevent personal injury.
- Recognize and identify limitation in providing care and seeks help from appropriate staff.
- Obtain vital signs twice a day am/pm times; once a month take weights measurements, and upon intake.
- Regularly check on residents to insure comfort, dignity and privacy; report accurate results to achieve the best possible member care.
- Learn and implement infection control policies.
- Recognize changes in resident’s condition and ask appropriate questions to clarify the situation; report changes to LVN/NP.
- Prepare member room for admission; provide member orientation to room and admission pack supplies
- Assist in toileting needs of residents and empty drainage collection devices
- Work in collaboration with other shelter staff to maintain smooth operation of the shelter, which may include assisting with food service or other duties as assigned
- Assist with meal preparation and implementation to ensure that all clients receive adequate nutrition
- Order office and program supplies when needed
- Facilitate client groups to assist with life skills development
- Develop effective, trusting relationships with clients using a client-centered approach that includes motivational interviewing, harm reduction and trauma Informed care
- Proactively create a supportive environment to ensure clients’ progress towards permanent housing and psycho-social rehabilitation
- Intervene during crises to actively de-escalate situations, ensuring safety for both staff and clients
- Ensure that the physical site is well-maintained and safe
- Send repair requests to Operations Department, when needed
- Communicate effectively, and in a timely manner, with management, co-workers, and clients
- Maintain client confidentiality at all times, while following agency, state, and HIPAA laws
- Current, unrestricted CNA license/registration in California
- Minimum one year experience as CNA
- Able to read, understand, and follow individual member care plans
- Able to lift and transfer residents into wheelchair, bed or care, etc.
- Able to obtain and record resident’s temperature, pulse, respiration, weight
- Able to assist with post-mortem care
- Able to meet all health, compliance and competency requirements
- Detail oriented with effective time management, organizational, written, verbal, and computer skills
- Knowledge of infection control procedures, including proper hand washing techniques, contact with blood spills, and standard precautions
- Minimum of one year experience providing services to individuals that have a history of homeless, mentally illness, or are dually-diagnosed
- Obtain and maintain CPR and first aid certification
- Bilingual in English/Spanish preferred
Work Environment
- Combination of field and office environment
- Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
- On occasion walk or drive to different local sites
- Will necessitate working in busy and loud environments
- Will be exposed to elements like cold, heat, dust, noise and odor
- May need to bend, stoop, twist and sit throughout the day
