Are you experienced, skilled, and passionate about making a difference to people’s lives? We’re looking for a dedicated Substance Misuse Practitioner to engage and assess people who use our service, ensuring they receive timely and personalised support that meets their individual set of circumstances and needs.
As a criminal justice drug and alcohol key worker you will manage your own small caseload of individuals accessing treatment for support to address their drug and/or alcohol use, that are actively involved in the criminal justice system. You will work in a all-inclusive way with people accessing our service, which will include communicating with other professionals, making referrals to other services, completing risk assessments and developing recovery-focused care plans which aim to support individuals to improve their health and well-being, achieve their personalised treatment goals, and reduce criminality.
This role will involve you working with individuals involved with the criminal justice system who may be in prison and shortly due to return to the community, or are already living in the community within the Chesterfield area. People on your caseload may be subject to Probation orders, community orders such as Alcohol Treatment Requirement (ATR) and Drug Rehabilitation Requirement (DRR), or may be involved with the Integrated Offender Management (IOM) team, courts, Liaison and Diversion and Prison healthcare teams. The role will involve some community outreach and prison in-reach work, working with a reduced caseload as you deliver a high level of intensive support.
