Position Title: Case Manager (Part-Time Employee)
Location: Corvallis, Oregon (with some travel in the Linn-Benton region)
Organization: Mid Willamette Trans Support Network (MWTSN)
Hours: 20 Hours or  0.67n FTE
Wage: $20–$25 per hour, depending on experience

About Us:
The Mid Willamette Trans Support Network is a trans-led grassroots nonprofit serving trans and gender-diverse people across rural Oregon. We believe in community care, mutual aid, and liberation work rooted in lived experience. Our work uplifts cultural connection, safety, healing, and access—especially for those most impacted by oppression and systemic violence.

Position Summary:
The Case Manager will provide direct support to trans community members by helping them access basic needs and navigate public benefit systems. This includes conducting intake and Health-Related Social Needs (HRSN) screenings, supporting enrollment in programs like the Oregon Health Plan, and connecting folks to affirming housing, nutrition, and other services.

This is a part-time employee position based in Corvallis, Oregon. The Case Manager will be responsible for maintaining a regular presence in our office and helping keep the space open and welcoming to the public. While most hours will be completed in-office during regular daytime hours, there is flexibility for occasional evening or self-scheduled work, especially when supporting clients or attending outreach events.

 

Key Responsibilities:

Client Support & Referrals

  • Serve as a consistent, affirming point of contact for trans and gender-diverse clients
  • Conduct intake assessments and HRSN screenings to assess needs around housing, nutrition, climate support, and more
  • Assist clients with enrolling in the Oregon Health Plan (OHP) and navigating programs like SNAP, SSI, and local supports
  • Coordinate referrals and follow up to ensure services are received
  • Check in with Microgrant recipients and help document outcomes
  • Ensuring data compliance meeting REALD and SOGI standards.

Resource Coordination

  • Maintain an updated, trans-affirming community resource list
  • Attend Gender Health Community Network Meetings on behalf of MWTSN
  • Build relationships with other service providers and local organizations

Documentation & Reporting

  • Maintain accurate, confidential case notes in compliance with organizational policies
  • Support with program data and reporting needs for grants and community outcomes

Community Engagement

  • Participate in outreach efforts and connect with community members who may not yet be linked to support
  • Support community events, workshops, and support groups hosted by MWTSN
  • Help facilitate food pantry and other support programs

Administrative & Office Support

  • Staff the MWTSN office during open hours, serving as a welcoming presence for walk-ins and phone inquiries
  • Support MWTSN leadership with general administrative tasks (e.g. organizing documents, tracking data, scheduling appointments)
  • Assist with managing supplies, event prep, and maintaining an organized workspace
  • Attend staff meetings and contribute to planning, coordination, and team communication

Required Qualifications:

  • Familiarity with Google Workspace (Docs, Sheets, Gmail, etc.)
  • Skilled with general computer software including Zoom, Microsoft Office Suite and Windows
  • Position Requires Travel, must have valid driver’s license and reliable transportation.
  •  Degree or relevant coursework in Queer Studies, Women/Gender/Sexuality Studies, Public Health, Public Policy, Human Development & Family Sciences, or related fields may replace degree with work experience

Preferred (but not required) Qualifications:

  • Lived experience as a trans, nonbinary, or gender-diverse person
  • Degree or relevant coursework in Queer Studies, Women/Gender/Sexuality Studies, Public Health, Public Policy, Human Development & Family Sciences, or related fields
  • Experience working or volunteering with nonprofits or grassroots groups
  • Familiarity with public benefits systems like OHP, SNAP, and SSI
  • OHP & SNAP Assister certification or willingness to complete training
  • Community Health Worker certification (increased pay available)
  • Strong commitment to trans justice, equity, and anti-oppressive care

 

Compensation & Benefits:

  • $20–$25/hour depending on experience
  • 30-hour work week
