Full-Time Primary Therapist (LCSW, LPC, LMFT, LAC)

Job Description

Mission: To facilitate group and individual therapeutic services for substance use disorder, mental health and trauma treatment to Women’s Recovery clients to increase their quality of life.

Outcomes (other responsibilities may be assigned):

Facilitates group and individual counseling

Counsels clients’ in-group sessions to help them cope with substance use disorder problems, mental health issues, and additional co-occurring addictive disorders and therapeutic issues according to curriculum

Intervenes appropriately for clients to resolve emergency problems in crisis situations

Maintains accurate, complete, confidential and HIPAA compliant records and documentation

Maintains accurate records and completes all documentation of group notes and individual sessions within 24 hours of providing service

Each provided service must be documented and signed by the primary therapist and noted on the chart checking and audit documentation to be reviewed by the clinical director, no later than the close of the business week each week

Maintains collaborative and accurate treatment plans and treatment plan reviews and session notes

Creates, at minimum, three treatment plans for each client on caseload: substance use, mental health and trauma, and family dynamics

Every treatment plan is collaborative and must be signed by the client themselves in addition to the primary therapist by the close of business each week

Session notes must be reviewed between the clinician and the client in collaboration during each individual session and signed by the clinician at the close of the session

Treatment plan reviews are collaboratively created and signed by client and clinician upon admission, at any transfer of level of care, or six months after last review, whichever comes first

Client obtains a copy of the treatment plan review upon completion after admission

Maintains concurrent documentation and trauma informed note taking – clinician will summarize session with the client at the close of the session, ask if they agree with the summary or have anything to add

Maintain standards and measures to show efficacy of programming and areas for improvement, creatively works with the clinical team to meet the following:

o Average Daily Attendance rate at standard expectation at facility level

o Against Clinical Advice discharge rage of <10%

o Completion Rate of >70%

o Length of Stay of 90+ days

o Optimization Rate of 90%

Remains knowledgeable on standards and expectations of the WR clinical department

Documentation handbook is memorized and utilized as a reference guide

Clinical handbook is memorized and tested on at the close of 90 day probationary period

Ensures they are utilizing best practice across the board and providing evidenced-based treatment modalities to all clients

Attend and implements findings from staff trainings

Attend outside trainings and opportunities as directed by the Clinical Director

Be knowledgeable on and executes appropriate documentation standards

Understands the importance of scope of practice, practitioner licensure and all ethical and legal requirements

Participates in activities that prepare for and maintain standards for all licensing, regulatory requirements, laws and best practices

Maintains accurate and up to date scheduling visible to team, supervisors and organization

Outlook calendar representative of weekly schedule of individual sessions, group sessions, crisis intervention, case management sessions, family communication and family sessions, and documentation blocks

Functions well and collaboratively within a team to provide the highest standard of client care

Participates in after-hours Crisis Intervention, responds to all calls and provides direction to support staff and clinical teammates when on call and is necessary

Actively participates in Clinical Staffing meeting and Clinical Supervision

Works with the admissions department to ensure a smooth transition into WR programming

Provides interview of client and/or family members when necessary

Provides a complete clinical assessment to individuals admitting to WR

Consistently reviews clients’ records for completeness and accuracy
