We are currently seeking a Patient Navigator to work at our 400 Broadway location.
Position Summary: The Patient Navigator will assist the Care Management Team Leader in working with clients to gather information to complete assessments, reassessments, specialty referrals, field -based outreach, etc. This individual will assist in carrying out assigned tasks that are needed to execute care plans that include; linking clients to primary care, accompanying clients to appointments when needed, assisting in obtaining health insurance, conducting home visits, assisting with other needed services/entitlements.
Qualifications: The Patient Navigator must be a high school graduate/GED with three (3) years of full-time work experience. Having an AA/AS in the related field with at least one (1) year of relevant work experience is preferred. This individual must have strong socio-cultural identification with the target population or experience in working with target population. Good written and verbal communication skills in English is required.
