PUEBLO REGIONAL CENTER (PRC) – PUEBLO WEST
PRC is a state run facility which provides supports to Colorado’s vulnerable population. We have 11 homes throughout the Pueblo West community. We provide supports for up to 88 individuals with dual-diagnosis, mental, and physical disabilities who are mandated under Colorado law to be in our care. Along with 24/7 living supports and care, we also run a Day Program during the week to provide meaningful skills, rehabilitation and work for our people supported.
Description of Job
Our staff operates, awake, 24/7, 365 days a year. We currently staff two shifts 7 AM-7 PM and 7 PM-7 AM. For approximately the first 45 days as a new staff, you can expect to primarily be scheduled between 7 AM and 7 PM. While we try to ultimately give staff a “home” to work at, it is important to learn all of the homes and know all of the individuals because as the need arises, staff will be moved accordingly – for the safety and support of our people supported and staff.Staff assists in all aspects of the day, from toileting and showering to accompanying individuals to outings, activities, and appointments. The people supported by PRC rely on the staff here to help them manage their day-to-day and work towards their transition criteria. When our individuals are asleep, graveyard staff performs duties such as continuing to monitor 1:1 individuals, placing grocery/laundry orders, doing laundry, performing deep cleaning, sweeping/mopping, preparing lunches, etc.Job Assignment
Under the direction of the Residential Coordinator (RC) and Registered Nurse (RN), the purpose of this position is to provide a safe and therapeutic milieu by providing safety checks, taking measures to maintain individuals/staff safety, and actively participating in programming for the assigned home(s). This position serves as a resource to other direct care staff, reviews and follows individual’s individual plans, ensures a person-centered approach, supervises residents during activities, ensures the nursing care (both behavioral and medical) is provided to all individuals, advocates for the individuals they serve, teaches self-help skills with a training approach, interacts with individuals in developing social skills, participates as a member of the multidisciplinary teams, educates families and other community co-members, and attends in-services. Position administers formal and informal programs within the scope of practice of this classification, provides immediate crisis interventions using verbal de-escalation and physical management techniques, responds to emergency situations, records and reports behavior and observations, and refers to RN for appropriate assessment.
12 hr Shifts:
- Days: 7am – 7pm
- Graveyard: 7pm -7am
Pool staff must be available to work all shifts. Attempts will be to made to keep you working the same shift, but there are no guarantees.
Compensation:
- $14.39 Hourly
- There is a shift differential for working graveyard and or weekend shifts.
Job Duties:
- Physically lifts, transports and/or positions individuals by using approved lifting techniques (i.e. two-man lift or pivot transfer).
- Provides assistance as needed to ensure residents’ needs are met in all areas of daily living including but not limited to; eating, hygiene, dressing, bathing, toileting and clothing care.
- Safely operates a motor vehicle to ensure safe transportation when transporting residents around campus or to outside locations, such as transporting clients to: work area, program area, medical appointments, community activities, and vocational training.
- Assists residents with personal shopping.
- Responds to behavioral challenges as needed with implementation of specific Behavior intervention programs (i.e., Safety Control or Emergency Control).
- Participates in the formulation of accurate information for assessments and formal reports for inclusion in the team process and individual plans (IPP).
- Maintains an attractive, well maintained living site by addressing safety concerns, making purchases, decorating living area and performing housekeeping duties to ensure a home environment. This includes assisting individuals with decorating their personal space.
- Fully understands all aspects of fundamental rights, including the right to be free from restraints and free of abuse.
- Understands the responsibility of reporting to the appropriate administrative staff any incident or evidence of abuse or neglect utilizing INotify and following PRC policies on reporting violations.
- Assures confidentiality of medical record and information per HIPAA regulations.
- Communicates and interacts positively with each individual we serve, visitors, family members, guardians, peers, and supervisors.
- Communicates well by actively listening, reading, reviewing, and understanding information and sharing that information by utilizing the following resources: State E-mail, Sign Me Book, Communication logs, Individual Plans, etc.
- Administers nursing care and positive supports.
- Serves as a resource to the registered nurse, advising and alerting them to any behavioral or medical needs.
- When necessary, intervenes in both medical and behavioral crisis situations.
- Provides services to all residents while meeting their needs with a training approach – allowing the most independence possible.
- Familiarizes self with individual’s allergies, sensitivities to food/drug interactions.
- Documents outcomes of program implementation in a timely and accurately manner.
- Records a wide range of information: residents’ responses to training, intake, output, menses, seizures, and all information relevant to all aspects of the client’s daily living and medical status.
- Prepares meals to prescribed diet consistencies as specified in the dietary planning menu.
- Documents variances and reports as directed by Dietary staff.
- Follows and maintains infection control standards and regulations.
- Follows and complies with all nursing competencies, and any classes suggested by supervisor.
- Maintains required training and certifications.
- Employs intervention techniques (Safety Care) and verbal techniques in addressing a behaviorally troubled individual who may be a threat to self or others.
- Completes, reports, and turns in paperwork in a timely manner for: incident reports, injury on the job reports, debriefing paperwork, progress notes, etc.
- Understands how to: obtain emergency assistance including after hours, respiratory services/oxygen, and equipment/laboratory services.
- Actively participates in fire drills and code zero drills.
- Adheres to organizations policies and procedures. Following nursing standard of care.
- Follows all standards of care as spelled out in the Pueblo Regional Center’s policy and procedures and complies with standards of regulatory agencies.
- Participates in agency work groups, committees and attends assigned meetings and in-services, training as scheduled.
- Attends mandatory and annual in-service training, as well as, assigned Cornerstone training to meet education and mandatory training requirements.
- Reports abuse allegations, claims of harassment/workplace violence immediately to the Emergency On Call Personnel (EOC) and Quality Assurance.
- Actively participates in the QAPI process at the home and Day program as directed by Leadership Team.
- Performs other duties as assigned
Minimum Qualifications:
High school diploma or General Education Diploma (GED).
Substitution:
Current, valid certification as a Nurse Aide (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED.
Current, valid licensure as a Practical Nurse (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact will substitute for the experience requirement.
Special Qualifications:
Valid, Current and unrestricted driver’s license
Preferred Qualifications:
High school diploma or GED
Previous experience working with individuals with developmental disabilities
Previous experience working with individuals diagnosed with mental illness or dual-diagnosis
Previous experience utilizing behavioral management and modification techniques
Capability to be trained and follow support and behavioral plans
Previous experience handling conflict resolution
Previous experience entering data into a computer
Good work history
Conditions of Employment:

- Required to report without delay or interruption to provide essential or emergency services to ensure health, safety, and welfare. Could be required to stay for multiple shifts due to inclement weather or staffing shortage.
- PRC is a 24/7 facility, therefore person in this position must be available to work any shift, any day and/or any area, to meet agency needs.
- Position will utilize a state vehicle to transport residents as needed. Position will travel between homes, CORE, run errands, and attend meetings, trainings, and seminars.
- Driver’s license must be valid and unrestricted and remain so throughout employment. Inability to drive a state vehicle, could result in a loss of employment.
- Training – must demonstrate the ability to lift 50 lbs, bend, kneel, squat, etc. and perform MANDT interventions proficiently throughout employment.
- Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
