Description of Job

This position exists to provide direct hands-on nursing care to facility residents to include assistance with activities of daily living, documentation of care provided, awareness of psychosocial and physical needs/safety of residents, all performed in a manner consistent with the standards of the profession, legal and regulatory requirements.

Primary Job Duties Include:

Resident Care

  • Provide direct resident care by hands on performance or assistance as needed with activities of daily living.  Provide environmental needs by arranging and adapting personal care items such as food or clothing and/or physical environment needs, e.g. wheelchair, walker, etc.  Gently guide and instruct residents as needed in self-care activities.
  • Assists, orients and serves as preceptor to new CNAs to the facility, residents, policies and procedures and routines.  The CCA II is a resource to the CCA I for suggestions and guidance in dealing with care for resistive residents.
  • Determine changes in resident’s type of care required; i.e., variance in resident’s individualized routine care due to change in resident’s condition or observation in health status.

Nutrition and Hydration Care

  • When required for specific resident, provides assistance with eating and drinking by cutting up food and feeding the resident, adding thickening agents to liquids, cue residents and remind them of the proper swallowing techniques and strategies suggested by therapist.
  • Documents percentages of food and fluids taken by residents on the hydration flow sheets.
  • Recommends observation of specific residents who may need dietary changes, e.g. from regular diet to pureed or regular liquid to thickened liquid.
  • Organize daily work so required care is provided despite unexpected occurrences to provide quality nursing care.

Safety

  • Monitor safety needs by supervising residents’ whereabouts, interactions and conditions and refer to charge nurse if necessary

Hygiene Care

  • Collaborate with charge nurse and other nursing personnel to provide and exchange information regarding resident care status, and to evaluate effectiveness and appropriateness of Treatment Care Plan, inform charge nurse of refusals of hygiene care.
  • Document observations and care provided on various residents records.

Psychosocial, Transport, Educational Requirements

  • Provide psychosocial needs of residents and their significant others by direct personal contact.
  • This position will be required to deliver lab specimens, drive a resident to a doctor’s appointment, or to the hospital (if not transported by ambulance) utilizing a State vehicle.
  • Maintain education by attending mandatory and non-mandatory in-services and workshops to keep current as mandated by state law and VA regulations and facility policy and procedures.

 

MINIMUM QUALIFICATIONS:

Education and Experience:

A high school diploma or GED  AND 1 year of experience as a licensed CNA

Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a pro-rated basis to determine qualifications. Substitutions:

Current, valid nurse aide license (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED.

Current, valid licensure as a Practical Nurse (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact will substitute for the experience requirement.

Special Qualifications:

Must have a valid Colorado CNA license upon entry into position.

Required Competencies

  • Skill in initiating, performing and completing assigned duties in providing care to residents.
  • Possess good verbal, listening and written communication skills with all health care providers, residents, families and multidisciplinary team members.
  • Demonstrated ability to display professional conduct at all times when dealing with fellow staff members, residents, families and the general public.
  • Demonstrated ability to maintain confidentiality at all times.
  • Long-term care/nursing home experience is highly desirable
  • Effective problem solving skills

Preferred Competencies 

  • Knowledge and ability to appropriately carry out assigned resident care based on the residents’ conditions; to use judgment and to accurately recognize, report and record relevant resident information.
  • Knowledge and ability to explain assigned resident care best practices to peers with less experience
  • Knowledge and ability to recognize urgent or emergent resident care situations, seek guidance and direction of the RN and/or Supervisor/DON, and initiate appropriate emergency interventions as directed.
  • Knowledge of quality standards, procedures, and protocols
  • Knowledge of specialized client care applicable to the work situation
  • Ability to effectively work with an resident population of varying physical, mental and emotional abilities and staff from multiple work units in the facility to achieve quality care planning and delivery

Conditions of Employment:

  • Must be able to work a flexible schedule including day, evening, and graveyard (night) shifts, as well as weekends, and holidays, as needed.
  • Must possess a Colorado Driver’s License.
  • Must pass TB test which will be given prior to employment at employer’s expense.
  • This position is considered essential services and therefore this employee must report to work without delay or interruption to provide essential or emergency
    services to ensure health, safety, and welfare of clients.
  • A pre-employment criminal background & CAPS check will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons. Felony convictions, conviction of crimes of moral turpitude, or convictions of misdemeanors related to job duties may disqualify you from being considered for this position. Should your background check reveal any charges or convictions, it is your responsibility to provide the CDHS Background Unit with an official disposition of any charges. Additionally, you will be required take and pass a drug screen.
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position.  Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
