We are looking for a licensed clinician (psychologist/PSY or licensed social worker/LCSW) to join our integrated behavioral health (IBH) team at AACI! Our team works within a Federally Qualified Health Center (FQHC) at two locations in San Jose, CA where we have a primary care clinic and psychiatry. We are currently in the process of growing our clinic and are looking to add another talented clinician.
We utilize the Collaborative Care Model to integrate behavioral health care into primary care in order to better address the needs of our patients and support their overall wellness.

Responsibilities of the position:
Provide warm hand offs for patients who endorse need for further evaluation (e.g., endorse symptoms of depression, anxiety, acute stressor, management of chronic medical conditions, etc.). Conduct risk assessments effectively and follow up accordingly.
Conduct intake assessments to better understand needs of patient, assess symptoms, and develop a diagnosis.
Develop effective treatment plans to target symptoms and track symptoms over time utilizing PHQ-9/GAD-7.
Provide individual and family to referred patients with mild to moderate symptomology.
Refer patients with moderate to severe symptomology to a higher level of care, which is often available within AACI’s specialty mental health program.
Collaborate with multi-disciplinary team (i.e., primary care providers, nurses, medical assistants, and psychiatrist) to coordinate care for each patient on your caseload.
Required Qualifications:
Current license as a Licensed Clinical Social Worker (LCSW) or Clinical Psychologist (PSY) in good professional standing
Preferred Qualifications:
Experience with providing various evidenced-based treatments (e.g., DBT, CBT, PST, ACT, etc.)
Ability to apply clinical skills and knowledge to the local community whom we serve
Excellent verbal and written skills
Clinical experience working with children, adolescents, and/or adults accepted. Our clinic provides primary care services to all ages.
