Full-Time Senior Diabetes Educator
Job Description
Position Summary
Under minimal supervision, the Senior Diabetes Educator will establish ADA recognition accreditation by developing and maintaining the Diabetes Education Database for the Nutrition and Wellness Clinic; perform Diabetes Self-Management Education (DSME) and nutrition assessment. Provide comprehensive analysis of the assessment results to clinic endocrinologists and referring providers. Support and assist in the coordination of follow up care for patients with Diabetes with complex medical needs.
Duties & Responsibilities
- Conduct research and analysis through the Clinic database to improve rates of basic survival skills and advanced DSME completion and accreditation and maintenance of NWC Diabetes Clinic ADA recognition. Participate in funded clinical studies related to diabetes care and delivery. Author and co-author journal articles.
- Assist in writing education grants. Monitor outcomes of DSME completion and Diabetes clinic attendance.
- Evaluate diabetes self-management skills in patients. This includes evaluating competencies in basic survival skills, advanced self-management of meals, exercise, sick day, hypoglycemic and/or hyperglycemic events.
- Monitor outcomes of DSME completion and present data for divisional and intuitional review. Participate in efforts to grow the clinical education program in volume and reach.
- Coordinate and lead diabetes education classes through the NWC education program.
- Perform other related duties and participate in special projects as assigned.

Minimum Qualifications
- Bachelor’s degree in Medical Dietetics and Certification in Diabetes Education.
- Minimum of five (5) years of experience in Dietetics and Diabetes education.
- Knowledge of formal screening and comprehensive nutrition and diabetes assessment tools, as well as to provide individualized parent education about ways to facilitate optimal diabetes care.
Preferred Qualifications
- Proficiency in Microsoft Office Programs (Access, Excel, Power Point, etc.).
- Proficiency in Chronicle Diabetes- DSM ES documentation platform.
- Strong written and verbal communication skills.
- Previous ADA accredited diabetes education program coordination strongly.
