Health Services – Long Term Care

Status: Regular Full Time – Contract Full-time 13 months

Salary Range: $89,469 – $111,834 per annum plus comprehensive benefits

Location: Peel Manor – 525 Main Street N, Brampton, ON, L6X 1N9

Hours of work: 35 hours per week, core hours 8:30AM – 4:30PM.

Flexibility to work some evenings and weekends is required to meet the needs of the Home. This includes participating in a rotational weekend manager on duty schedule.

 

Position Summary:

 

Manages all aspects of the Social Work scope and responsibilties within the centre; this includes managing, organizing and overseeing the entire admissions, internal transfers and discharge processes. The individual is responsible for resident interventions related to their social and emotional well-being, provision of emotional support to families and liaison with community resources. All of the interaction with residents and families is in conjunction with all other disciplines working in the centre.

 

What you will do in this role:

Reviews applications/updates received. Reviews all applications to determine waitlist and admission suitability, reviews with SOC/DOC/Administrator/ and appropriate members of the care team as needed
Working with the multi-disciplinary team, interprets and analyzes complex information
in order to provide recommendations around behaviour supports and dementia care
Identifies and provides recommendations to DOC/Administrator/ ADS supervisor regarding waitlist refusals
Liaises with Ontario Health atHome for appropriate resident admissions from the waiting list, monitors the waitlist while managing the homes internal transfer list. Consults with the DOC/SOC’s/Administrator and coordinates the internal transfer process (i.e., for reasons of responsive behaviours, exit seeking or to ensure residents are residing in the appropriate type of accommodation)
Interprets and analyzes complex information in order to provide recommendations around behaviour supports and dementia care
Provides expert guidance and education on the interpretation of the Ontario Health Care Consent Act and Advance Care Planning
Coordinates the removal of resident/client personal property at discharge with other departments.
Conducts weekly and pre-admission tours
Acts as the key representative for the home’s Palliative Care program and serves as co-chair with the Pain/Palliative Care/Ethics Committee where appropriate
Provides ongoing therapeutic counselling and follow-up support to residents, clients, families and staff related to admission, discharge planning, internal transfers, conflict resolution, disease process, palliation, bereavement and Substitute Decision Makers
Skilled in crisis intervention and handling complex resident and family situations
Participates in the transdisciplinary team process of resident assessments including cognitive assessments, care planning and evaluation to ensure services are provided according to individual needs
Provides expert guidance and advice based on assessment of resident needs and concerns
Completes documentation as required in the health record and in accordance with the College of Social Worker and Social Service Worker standards
Writes reports, prepares correspondence and maintains case documentation. Identifies, analyzes and provides recommendations on trends, risks and opportunities in area of speciality
Determines, develops and recommends treatment plans based on the values of the organization considering privacy, cultural diversity, intimacy, dignity and quality of life
Co-ordinates referrals and serves as liaison with internal/external programs, services and agencies.
Provides expert interpretation of Legislation/Acts, internal policies and procedures and advocates on behalf of residents and families with internal and external stakeholders including matters related to Ethics and Capacity
Develops and maintains effective relationships with internal and external stakeholders including those in alliance with the strategic plan
Ensures compliance with relevant Regional, and Provincial Acts and Standards, Accreditation Standards and Ontario Health Agreements
Participates in multidisciplinary decision-making within teams and on a one-to-one basis
Attends resident related care conferences including: initial, annual, palliative and situational conferences
May promote the centre and attend public relations activities including public speaking engagements
Coordinates events where applicable i.e. seniors’ month, Elder Abuse Awareness Day, Family Council Week, etc.
Acts as staff liaison for Family Council meetings as required
Maintains effective communication with the Administrator and Management Team members
Active member of the Centre’s Management Team and provides weekend supervision as required
Participates as a member of the Social Work Peer Group and attends meetings with peers from other Regional long term care centres as required
Supervises and evaluates Social Work/social service worker student placements
Researches, develops and facilitates presentations for family council
Assists supervisors by providing work direction, advice, training and or guidance to staff, residents and families
Performs other duties as assigned in a safe and healthy manner
What the role requires:

A Bachelor of Social Work degree from a recognized university and membership in good standing with the Ontario College of Social Workers and Social Service Workers
Additional courses in gerontology
5 years’ experience
Demonstrated skills in assessment, facilitation and counselling
Counselling experience in a long-term care centre or social service agency that serves the elderly.
Knowledgeable of the psychosocial needs of the long-term care population
In-depth knowledge of available services/programs to assist clients and their families
Develop strategic relationships in order to navigate health and social service systems
Knowledge of gerontology, Alzheimer disease and related dementias
Knowledge of relevant Regional and Provincial legislation including the Fixing Long Term Care Homes Act (FLTCA), Health Care Consent Act, Substitute Decisions Act, MFIPPA, PHIPA policies and procedures and other provincial acts
Current knowledge related to the Consent and Capacity Board and Powers of Attorney
Prior community development/ work experience in community services with the elderly population
Strong communication skills both written and verbal to be able to effectively work with residents, families, staff, volunteers and all internal and external stakeholders
Effective conflict resolution skills to assist residents and family members
Leadership skills to motivate; and to provide coverage in the absence of the other management team members including weekend supervision
Public speaking skills to promote long-term care centres
Strong problem-solving skills to be innovative and creative in developing strategies that successfully meet the needs of residents and families
Strong organizational/planning and time management skills
Interpersonal skills to work effectively with residents, staff, volunteers and students
Budgeting skills to assist in formulating and administering budget and record details of expenditures such as Family Council funds, resident purchases and special events
