Job Description
RECONNECT aims to address the health inequalities experienced by those released from prison. The RECONNECT worker supports them to make the transition to community-based services that will provide the health and care support.
The right candidate will provide specialist person centred support to men with substance misuse issues and or mental health issues on release from prison into Hertfordshire. The Hertfordshire RECONNECT service enables service users to access and sustain supportive interventions upon release from prison, whether that be physical or mental. The service will offer, 1:1 support, sign-posting and onward GP/Health and aftercare referrals and support.
We are looking for an enthusiastic individual who will be responsible for processing client referrals and assessing and supporting client needs. The individual will need to be well organised and able to build and maintain relationships with internal and external stakeholders. You will be required to travel across the county and be flexible to service need. The role itself will be a mixture of WFH and being outreach/community based.
