Full-Time Casework Coordinator

Job Title:                  Casework Coordinator

Service:                    Devon Supported Housing Service

Location:                  Exeter

Salary:                      £22,440- £23,812 per annum

Hours:                      Full-time 37.5 hours

Working Pattern:     5 days a week, participate in an out of hours call rota.

Responsible to:       Service Manager Devon Supported Housing

Additional info;   

  • On-call (1 in 7 weeks), £100/week when on-call
  • This role requires the candidate to have a car (Parking paid if visiting different projects during the working day)

 

Responsible for

The Casework Coordinator is expected to hold day to day responsibility for managing referrals, assessing and supporting clients in a way that allows them to maintain accommodation, manage personal finances and make positive changes in their life.

Key Responsibilities 

  • Build positive and effective working relationships and agreements with relevant external agencies. 
  • Progress and track referrals made into the service and outcomes for service users moving on from service. 
  • Complete needs assessment/risk assessment for clients leading to the delivery of key elements of support plans which meet individual clients’ needs and self-selected aspirations.  
  • Give clients emotional support, information and provide an enabling service so that they can be signposted to and access appropriate services.
  • Participate in an on-call rota system providing support to clients outside of core working hours.

Person Specification 

  • Understanding of the causes of homelessness and social exclusion and approaches which seek to address these
  • Understanding of trauma and domestic abuse/victim support
  • Ability to undertake Needs Assessment and Planning for people with complex social needs which includes taking a lead on multi agency working and partnership
  • Working with people with complex needs and challenging behavior
  • Have a full driving license and access to a car for work purposes
