Job Description:
Easterseals of Louisiana has a new partnership and we need a dynamic person to join our team and help us build this new program. We are seeking a Case Manager – Rapid Re-Housing Program. - Apply the Housing First approach to serving people with housing and other service needs.
- Will engage high-risk individuals with a focus on successfully linking them to needed higher level of care.
- Participates as an active member of Unity of GNO’s Service Providers and Professionals Association (SPPA).
- Submit routine reports and outcome data, per contractual requirements, will utilize HMIS system.
- Attend and participate in weekly staff meetings.
- Attend and participate in required training.
Qualifications (please take note, we have no flexibility on degrees accepted. These are conditions of our contractor):
- a Bachelor’s Degree in a Human Services related field (i.e. Sociology, Psychology, Education, Counseling, Social Services, Family & Participant Science, Rehabilitation Services, or equivalent.)
- Reliable transportation so that you may make visits to the field to conduct participant visits. You may anticipate about 50% or more of your time will be in the field visits.
- Easterseals Louisiana is a Drug-Free Workplace, pre-employment and random drug tests will be administered.
- Will be required to pass Easterseals Louisiana background check
