Full-Time Coordinated Entry Outreach Specialist
Job Description
SUMMARY: The Coordinated Entry Outreach Specialist administers a range of data entry and Coordinated Entry referrals for the Coulee Collaborative to End Homelessness. Follows direction from Coordinated Entry Manager to ensure homeless participants have access to the Coordinated Entry System. Works with the Homeless Management Information System (HMIS) to enter data and manage referrals to the Coordinated Entry System. Assists case managers in securing coordinated entry documents for client files in housing programs. Employee must be able to exercise discretion in handling confidential matters.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Supports and upholds Couleecap’s mission to fight poverty and promote self-sufficiency for individuals by helping to identify needs, mobilize resources, and provide quality services.
Conducts referral process for Coordinated Entry participants to include the completion of the HMIS Release of Information, the Wisconsin Balance of State Continuum of Care (WIBOSCOC) Rights and Responsibilities, WIBOSCOC Screening Form, Vulnerability Index – Service Prioritization Decision Assistance Tool (VISPDAT), homeless episodes tracking form, and documentation of disability. Enters all Coordinated Entry referrals into Clarity (HMIS) and provides regular follow-ups to ensure all Coordinated Entry requirements are fulfilled.
Maintain and conduct follow up on referrals made to the Coordinated Entry System.
Attend all Coordinated Entry trainings and ensure best practices are followed.
Work with area agencies in the Coulee COC to develop and implement the After-Hours Plan. Along with committee members in the Coulee COC, ensure the After-Hours Plan is reviewed regularly and is current at all times.
Serves as agency user for Clarity Database. Accurately enter data in order to ensure 100% data accuracy.
Greets general public, clients, vendors, and other business personnel, directing them to appropriate staff person and/or programs within the agency. Answers telephone and manages messages to staff. Refers individuals to other available resources/services in the community.
