Full-Time Step Down Co-ordinator

YMCA DownsLink Group are currently recruiting for a Step Down Co-ordinator to join our team at St Patrick’s to provide support to clients residing in short-tem respite beds after they have been discharged from Royal Sussex County Hospital.
The successful candidate will line manage two personal assistants who provide daily support for clients using the Step Down service and will work closely with the staff team at St Patricks to coordinate their care and support needs. You will take a lead role in all operational aspects of the Step Down service, including working closely with relevant organisations (Arch Healthcare, RSCH Pathways team, CGL, Justlife, etc).
You will coordinate support for people moving into the Step Down service, as well as coordinating plans to move people out of the service within a target timeframe of 5-6 weeks through 1-1 work with clients, support planning and risk assessments, attendance to Step Down Operational and Move On Meetings and completion of Wellogram, Move-in and Move-Out feedback and evaluation questionnaires.
Key Requirements
- A strong team player who can support clients and colleagues appropriately
- Proven experience in safeguarding adults who may be or are at risk of harm
- Experience working with vulnerable adults in a Trauma Informed, Psychologically Informed and Asset based way.
- Experience of supporting clients to engage in extra and specialist support, incl. referrals to external agencies and signposting.
