Full-Time Early Intervention Services (EIS) Specialist

Job Description

Position Purpose

With the guidance of the Testing and Linkage to Care Manager, the Early Intervention Services (EIS) Specialist will assist newly diagnosed and out-of-care individuals in accessing primary HIV medical care, conduct outreach and case finding activities with high-risk focus populations, and facilitate HIV education to individual clients and in group settings.

Essential Functions

  • Perform street outreach at least twice per week
  • Conduct street outreach with specific intentions to connect with hard to reach populations, including but not limited to, individuals experiencing homelessness and/or who were recently released from incarceration.
  • Assist newly diagnosed and out-of-care individuals with linkage to medical care and referral to case management services and other social services.
  • Provide transportation and attend medical appointments with clients
  • Advocate on behalf of clients and provide referrals for other services in the community, such as mental health support, substance use treatment, housing assistance, and SOAR.
  • Coordinate with area hospitals, stand-alone urgent care centers, and community facilities to ensure that a partnership is in place for client referrals into services.
  • Maintain documentation and reporting requirements, including case notes, in the client record according to departmental standards by utilizing a computerized information system.
  • Provide education and risk reduction to both HIV positive and negative individuals.
  • Facilitate HIV 101 classes.
  • Have access to a reliable vehicle and maintain valid driver’s license
  •  Perform other duties as assigned.
Required Qualifications

  • High school diploma or GED required
  • Valid driver’s license required
  • Must have and maintain valid driver’s license, liability insurance and safe driving record
  • Ability to demonstrate good judgment, maintain strict confidentiality standards, and adhere to professional standards
  • Ability to make sound decisions in accordance with organization’s policies, procedures, and guidelines
  • Ability to prioritize and manage multiple tasks
  • Ability to work in a team environment

Preferred Qualifications

  • At least one year of experience working in an outreach capacity
  • At least one year of experience with mental health, substance use, or HIV disease/prevention
  • Experience working with individuals and communities of diverse cultures, ethnicity, socioeconomic backgrounds, sexual orientations, and gender identities and/or expressions
  • Bilingual (English and Spanish)
