TRANSITION WORKER (maternity cover)
£25,238-25,845 per annum,
37 hours per week
The working pattern is Monday to Thursday 7.5 hours per day, Friday 7 hours per day
52 weeks per annum,
Abingdon Campus
Start date December 2023
This is a maternity cover and is fixed term until approximately 20th January 2025
We are looking for expressions of interest for maternity cover for a Transitions Worker to provide a link between schools and parents within the local area to enable young people with SEND to make successful transitions to college.
We are proud of the partnerships that we create with local schools, employers and the Local Authority agencies to support learners with SEN and/or disabilities to secure positive transition to college, thereby enabling the students to access a
positive learning environment and opportunities to develop the skills to live and work independently.
You will co-ordinate transition for students accessing Vocational courses as well as Gateway and Fast Forward. This will involve developing relationships with key stakeholders including the Preparing for Adulthood Social Care teams, the Local
Authority and other professionals central to the student’s Education and Healthcare Plan (EHCP). You will successfully promote the college study programmes and SEND support networks to build confidence through every step of the
transition process.
This would suit an existing member of staff who is looking for a fixed term opportunity and therefore available as a secondment from an existing post.
