Full-Time Telehealth Case Manager
Job Description
Full-time Telehealth Case Manager position requiring an individual who is a self-starter with exceptional communication skills to effectively engage with and establish a rapport with patients across existing community treatment teams. Position has no pre-assigned caseload; patients may range from young adults to elderly individuals.
This position will be 100% home-based and work entirely remote upon the successful completion of the Introductory Period, phasing, training and demonstrated consistent productivity maintained at 104 billable hours monthly (1,248 annually).
No driving is permitted of this position for purposes of community-based treatment or patient transport services. Mileage reimbursement is not applicable.
WHAT YOU WILL DO:
- Coordinate patient services across teams as needed
- Prepare Individual Service Plans for treatment after expiration or as requested
- Coordinate and identify housing resources and prepare applications with patients as necessary
- Prepare Outcome Questionnaire (OQ) surveys
- Provide other billable services as assigned
- Update the electronic medical records software with current patient information in order to maintain accurate records and submitting your electronic case file notes daily for billing purposes.

QUALIFICATIONS:
- Must be an Ohio resident
- QMHS – BS/BA Required to meet State of Ohio Therapeutic Behavioral Service requirements
- Motivated, organized and detailed personal traits
- Exceptional communication skills
- Knowledge of community resources helpful
- Prior exposure or experience to mental illness or prior case management experience preferred
- Must complete 90-day Introductory, Phasing and Training program
- Must meet minimum productivity requirements for billable services at 104 hours/month (1,248 annually)
- Pre-employment criminal background check and drug testing are mandatory
