- Expected starting salary is $31.84 – $42.51. Full salary range is $31.84 – $53.18.
Bachelor’s or Master’s degree from an accredited college or university.
Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver’s license and be free of any major traffic violations for the last three (3) years.
Desirable Qualifications:
Two years of social service-related experience.
- Nature of Work
- Under supervision of a Program Manager, and within Federal, State and locally developed programs and policies, reviews referrals for State Plan and Waiver Services, provides Long-Term Care Consultation, and completes MnCHOICES Assessments and Reassessments. Eligibility for home and community-based services and medical assistance services is evaluated.
- Examples of Work
- Reviews referrals through intake process. Gathers information about functional abilities, medical information, and level of care for services.
- Provides assessment and occasional case management for individuals served through State Plan, Waiver Programs, and Medical Assistance Programs.
- Conducts MnCHOICES assessments, reassessments, courtesy screens, and county of location reassessments for individuals applying for State Plan or Waivered Programs and other related services.
- Assesses the functional needs and abilities of the individual to determine eligibility for services.
- Interviews client/caseloads to determine eligibility and identify need for service/programs.
- Develops and writes individual service plans, community support plans, etc.
- Utilizes State and Local computer programs to document work for the county and the individual.
- Provides information about health care access and assists individuals to access Medicare and Medical Assistance funded health care services.
- Completes required contacts and paperwork within established/required timelines.
- Advises clients of community resources, information and referrals for services, rights and responsibilities under programs or services.
