- Minimum Qualifications of Education and Experience
- Expected starting salary is $31.84 to $42.51. Full salary range is $31.84 – $53.18.
Bachelor’s or Master’s degree from an accredited college or university.
Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver’s license and be free of any major traffic violations for the last three (3) years.
Desirable Qualifications:
Two years of social service-related experience.
- Nature of Work
- This position provides consultation and referral services to individuals, families, and target groups in the community. Works primarily within the Adult & Family Services Division with work assignments to include Care Coordination for Managed Care Organizations for individuals 65 years and older.
- Examples of Work
- Provides assessment and care coordination for individuals 65 years and older
- Interviews client/caseloads to determine and identify problems or needs for service/ programs.
- Develops and writes individual service plans.
- Provides information about health care access and assists individuals to access Medicare and Medical Assistance funded health care services.
- Provides information about managed care health benefits.
- Completes required contacts and paperwork within established timelines.
- Advises clients of community resources, information and referrals for services, rights and responsibilities under programs or services.
- Performs related duties as assigned.
- Knowledge, Skills and Abilities Required
- Considerable knowledge of the social sciences
- Knowledge of home and community-based services and supports.
- Knowledge of social and economic problems
- Knowledge of motivating factors of human behavior
- Knowledge of the principles of social work
- Some knowledge of community resources
- Knowledge of federal and state welfare programs and relevant laws
- Knowledge of agency policies
- Skilled in verbal communication, public speaking, writing and recording of information and data
- Ability to consider clients’ problems with empathy and objectivity
- Ability to exercise good judgment in recommending possible solutions for client’s difficulties and when utilizing conflict management and advocacy skills
- Ability to establish and maintain good working relationships with clients, co-workers, public officials and citizens of the community
- Ability to organize efficiently and keep records systematically
- Ability to complete MnCAT Certified Assessor Training to become a MnCHOICES Certified Assessor
- Ability to work independently and to recognize when supervision is needed
- Ability to drive an automobile
