This is an excellent opportunity for a Registered Nurse- Gender Affirming Care (40 hrs. /wk., benefits eligible).
PURPOSE of POSITION:
Under the direction of the Associate Director of Clinical Services and Gender Affirming Hormone Therapy Program Director, the RN in Gender Affirming Care Services will be responsible for providing medical follow-up to patients based on clinicians’ management plans, conducting telehealth and in-person injection training visits, communicating with pharmacies and insurance companies regarding patients’ prescriptions, and answering and returning medical-related patient inquiries. The RN in Gender Affirming Care will act as a care coordinator for patients needing care inside and outside of PPWI. The RN will also provide coverage to health centers on an as needed basis.

·         Ability to handle sensitive information in a confidential manner.
·         CPR certifiable.
 
EDUCATION and/or EXPERIENCE:
·         Must have a current license as a Registered Nurse by the State of Wisconsin.  Bachelors of Science degree in nursing preferred.
·         Minimum of one-year related work experience in gender affirming care or working with the LGBTQ community preferred.
PHYSICAL REQUIREMENTS:

Physical demands of the position necessitate hearing, talking, handling and grasping regularly.  May require lifting up to 30 pounds. Specific vision conditions include close vision reading. Requires the ability to operate a computer, telephone, fax, copy machine and adding machine or calculator.  Requires the ability to operate medical equipment.
WORKING CONDITIONS:
The position is generally performed in a medical office environment but involves frequent interruptions.  The noise level is generally low to quiet.  The work environment characteristics are representative of those an employee encounters while performing the essential functions of this position.  Reasonable accommodations, however, may be made to enable individuals with disabilities to perform said essential functions.
Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment. Will work with blood or blood-borne pathogens and will require OSHA training.
This position requires occasional traveling, therefore a valid driver’s license and auto insurance in accordance with agency liability standards is required.
Willingness and ability to work a flexible schedule, including evenings and weekends.
  • Daily review of Provider Approval Queues (PAQ) to include accepting and signing off of normal and abnormal laboratory results defined in PPWI protocols, policies and procedures.
  • Responsible for creating and/or managing patient-related follow-up plans per standing orders for abnormal test results and referrals, task all others, place alerts and start notifications as needed and maintain next action date tickler system.
  • Provide medically accurate education for all laboratory & medical results as assigned and accurately document in the medical record/plan.
  • Organize lab services when patients are getting labs drawn at an outside facility and enter results from faxed labs into EHR.
  • Seek consultations as necessary from affiliate clinicians and GAHT Program Director.
  • Generate referrals for patient care.
  • Generate support letters for patients seeking gender affirming surgeries and name/gender marker changes.
  • Responsible for assessing, planning and administering health care which is individualized to the patient and consistent with the policies and protocols of PPFA & PPWI in person and through telemedicine and patient phone calls.
  • Accept and manage tasks, make contact attempts and provide education materials as required per protocol.
  • Run daily reports from electronic health records to determine next action steps in follow-up.
  • Review medical records from external providers related to patient’s follow-up care or medical/record plan.
  • Answer and return phone calls and tasks from patients, pharmacies and other entities; check voicemail throughout the day, return appropriate calls and document client information from calls.
  • Complete pharmacy related tasks within scope of practice and affiliate protocols; reassign pharmacy-related tasks that are outside RN scope of practice to clinicians.
  • Work closely with Gender Affirming Care Patient Navigator to most efficiently aid patients with their needs based on individual roles and availability including tasks such as providing resources outside of PPWI, completing prior authorizations, and helping patients schedule appointments within PPWI.
  • Work closely with clinicians trained in GAHT to complete tasks for GAHT patients within RN scope of practice.
  • Meet metrics for volume/time for reminder letters and contact attempts.
  • Maintain ongoing and appropriate communication with all staff in gender affirming care services as well as the patient services team.
  • Participate in agency quality assurance program. Under the direction of the Quality and Risk Manager, implements overall quality assurance activities including audits.
  • Assist with on-call services according to agency protocols.
  • Staff telehealth for Gender Affirming Care within scope of practice including injection training visits.
  • Train new reproductive healthcare assistants in the provision of GAHT services.
  • Complete other duties as assigned.
