Hertfordshire RECONNECT Service
RECONNECT aims to address the health inequalities experienced by those released from prison. Based across Hertfordshire, the service enables service users to access and sustain physical and mental health supportive interventions upon release from prison.
The Role
Working Monday to Friday, this role supports people to make the transition from prison to community-based services that will provide health and care support.
Through face to face meetings, in prison and after release, you will provide 1:1 specialist person centred support to men with substance use and mental health issues. You will be responsible sign-posting, processing referrals and assessing and supporting client needs.
Hybrid Working
A large part of the role is working across Hertfordshire in prisons such as HMP The Mount and in the community. You will be required to travel across the county and be flexible to service need. You will work from home approximately 10-20% of the time. Travel expenses covered under our expenses policy.
About You
· Relevant experience of working in health and social care, ideally within the field of problematic substance use, mental health or another comparable field
· Passionate and enthusiastic about making a real difference to the lives of people we support
· Evidence of effective partnership working and the ability to network for the benefit of the client and service
· Evidence of managing a busy caseload or tasks and excellent organisational skills
· To be proficient in the use of Microsoft Outlook and Word and be familiar with database management systems
· Excellent communication skills, both written and verbal
