- Raising AWARENESS about the need for families for Minnesota children
- Providing INFORMATION AND REFERRAL to those interested in becoming adoptive, foster care, and kinship care families.
- Offering EDUCATION relevant to adoptive, foster and kinship families and the professionals that work with them.
- Providing individualized SUPPORT, GUIDANCE and RESOURCES to foster, adoptive and kinship families.

MN ADOPT is currently hiring for a Post-Adoption Navigator. This position is a full-time (40 hours per week), Monday-Friday day time hours, and Metro Based (St. Paul, MN). This role, which is a part of the MN ADOPT HELP Program (www.mnadopt.org/help-prog/), entails the following:
- Provide phone, mail, and email-based outreach to adoptive and kinship families at the time of finalization of adoption from MN foster care to help increase awareness of the network of resources and support available and to assist with answering questions the family may have.
- Work in partnership with the MN Department of Human Services to receive family contact information and conduct timely outreach to all families who have recently finalized in Minnesota.
- Help to normalize the need for and importance of post-adoption support through clear verbal and written communication.
- Provide reliable and responsive support including linking families to adoption-competent resources and permanency support services as needed.
- Track family outreach efforts, document communication and resource referrals provided to all families, and conduct program evaluation measures to help assess program outcomes.
- Work closely with the HELP Program Manager and collaboratively with teammates, other MN ADOPT programs, outside organizations, county systems and other permanency support service professionals.
The following factors will be considered in evaluating candidates:
- Bachelor’s or Master’s degree in Social Work, Psychology, a related field, or equivalent experience.
- Minimum of two years of direct service experience with the child welfare system preferred.
- Professional and/or personal experience with adoption, out-of-home placement, trauma and attachment considerations across stages of development and family systems.
- Knowledge of Minnesota’s child welfare system, human services and programs, and permanency support services available to adoptive and kinship families after finalization.
- Strong organizational and communication skills; ability to effectively engage with caregivers, parents, professionals, and members of the community with diverse backgrounds, perspectives, and circumstances.
- Team oriented and demonstrates enthusiasm in taking an active role in ongoing program development and evaluation.
- Proficiency and comfort with computers and technology.
