Full-Time Intake Assessment Counselor
Job Description
Counselor to conduct intake assessments and re-assessments for behavioral healthcare company headquartered in Short North area. Workweek is Monday-Friday, 8:00 a.m. – 5:00 p.m. No weekends, holidays or on-call schedule is required. Position is full-time, non-exempt, paying $25 hourly.

KEY FUNCTIONS:
- Conduct mental health and SUD assessments and re-assessments with patients to meet OMHAS, CARF and agency standards
- Assist with completion of all documentation required for Intake
- Review patient records
- Update patient records and information
- Assist staff in providing treatment services prior to or following assessments
- May consult or interview family members as well as other community members in order to arrange patient assessments
- Assist in completing case terminations
QUALIFICATIONS:
- Requires a Bachelor’s level education, at minimum, and licensed within Ohio as an LSW, LISW, PC, PCC, MFT or IMFT.
- Prior SMD or SUD experience;
- Prior experience conducting assessments;
- Must meet minimum requirement of 1,020 billable hours annually (85/month);
- Must meet company credentialing requirements;
- Valid Ohio driver’s license, automobile and excellent driving record are required; and
- Pre-employment criminal background check and drug testing are mandatory
