Job Description

HIV Program Manager

The HIV Program Manager oversees all programmatic activities and operations of HIV services at Erie Family Health Centers.  The Manager is responsible for providing supervision and training to direct staff and team members across the agency, on-site coordination of wrap-around services, managing external partnerships and relationships, monitoring program quality, working collaboratively with providers and care team to integrate case management with clinical care. This position also works collaboratively with Patient Program Managers and other leadership to achieve program requirements (completing reports, program planning, grant applications, and audits) and implement department initiatives. The ideal candidate will have a strong commitment to working with an underserved population and a desire to increase access to medical care and resources for people living with HIV. 

Responsibilities:

  • Oversees all programmatic activities and operations of the Lending Hands for Life program including performance and patient satisfaction
  • Manages program budgets, grant applications, reporting, and all program compliance needs
  • Monitors Provider Enterprise database system and EMR for quality assurance and program preparation for audits and site visits
  • Partners with Leadership to manage all agency HIV testing services, perform clinical chart audits, create workflows aimed at process improvement, and assist with the integration of the HIV case management programs/services with the clinical care team.
  • Provides supervision, professional development and mentorship, training, performance management of direct reports, and assessing need for staff development
  • Assists with coordinating and promoting on-site services and to execute department initiatives as needed
  • Partners with Process Improvement team and Medical Director on planning, co-leading, and facilitating monthly QI committee and annual QI plan
  • Manages key strategic partnerships with external agencies, attends all agency and grant required meetings, trainings and/or conferences
  • Provides regular and ongoing communications to providers and agency leadership regarding programs and service

Requirements:

  • Bilingual fluency (English/Spanish) required
  • Bachelor’s degree in community health or related field required (Master’s Degree in Public Health, Social Work, or related field strongly preferred)
  • 3 years of experience working in a HIV program or related services
  • 3 years of management experience
  • 3 years of experience in a health care delivery organization.
  • Computer literacy required and Provide Enterprise database system knowledge preferred
  • Experience developing and tracking budgets, developing and implementing outcomes-based work plans, evaluating and monitoring program efficacy
