Position Description

In this role you will be responsible for:
- Develop policies, procedures, and protocols to systematically increase oral health care utilization and access among HIV patients.
- Partner with leadership and staff in Dental Clinic and IDC to create referral process and enhanced patient access to services.
- Create systems for data tracking for funding requirements and billing activities.
- Engage HIV positive patients in oral health care.
- Collaborate with dental and HIV providers to coordinate comprehensive care.
- Provide patients with education on HIV/AIDS disease acquisition, treatment and prevention services.
- Coordinate professional education for dental residents, providers, and staff on screening, assessment, and treatment of HIV; coordinate professional education for HIV providers on oral health care screening, assessment, and services.
- Revise intake tools and program brochures and materials to include dental and HIV care in each practice.
- Enhance positive healthcare outcomes and community awareness about HIV/AIDS, impacting community health through education, linkage, referrals and direct service delivery.
- Identify and evaluate models that transform sporadic users of health care into regular and continual users of healthcare; and identify effective methods to support and retain clients in care.
- Track progress using Careware database and MS excel as appropriate; use data to identify needs assessment.
- Provide ongoing adherence and retention services and psychosocial support.
- Assist patients in linkage and navigation of the healthcare system; insurance paperwork and co-pay assistance enrollment.
- Disseminate program results and best practices.
- Comply with reporting requirements.
- Participate in discussions in staff meetings and case consultations;
- Promptly attend scheduled staff meetings, in-services and trainings.

- This position requires providing service to the following age population(s) 13-80 in a manner that demonstrates an understanding of the functional/developmental age of the individual served.
If you possess the following qualifications, please apply immediately:
- Accredited Social Work or Public Health Program graduate with experience providing education, support, and program referrals to HIV infected/affected persons. (required)
- Knowledge of HIV infection related social and emotional issues, and demonstrates competency in working with culturally diverse, low income, or no income clients and special populations. (required).
- Experience in assisting clients in social services and healthcare access, as well as knowledge and experience in providing HIV/AIDS education and information is preferred, including working knowledge of medical terminology particularly as it relates to HIV disease preferred.
Physical Demands:
- Physical Requirements: Physically active, able to walk in neighborhoods (to homes).
- Visual Requirements: Must be able to see charts, read numbers from a distance, and read small print and have visual dexterity to work with computers.
- Hearing Requirements: Able to hear client?s comments and concerns, able to use telephone for conversations with clients.
- Working Conditions: To share office with other staff; however most often in the community
