CHS Healthcare are looking to recruit a Healthcare Coordinator, who will provide first point of contact and administration duties, within our communications and domiciliary care team. The role will you see processing referrals and working cross-functionally across the wider team.
Part of Acacium Group, CHS Healthcare is the UK’s largest independent provider in hospital discharge management and continuing healthcare services across all four NHS regions in England. We have been a trusted provider to the NHS since 1995, bringing together the NHS, social care, providers and families to provide a and patient focused service.
Every day you will…
- Source both short term and long-term care home placements/packages of care for clients working towards a quick, safe, and correct discharge plan for patients with a variety of needs; including complex physical disabilities, challenging behaviour, and mental health
- Liaise with care homes, care agencies, senior health and social care staff, patients, and their families
- Meet with families often at a great time of stress.
- Share updates and act as the link between discharge teams, local authority, clients, ward nurses, care providers and service users, ensuring the discharge happens within set timescales
- Visit care homes and build relationships with Home Managers and their teams

To thrive in this role, you must ideally have…
- Experience within customer service and administration
- Excellent written and verbal communication skills
- Access to a vehicle and a driving license
- Experience using Microsoft packages such as Word, PowerPoint, and Excel
