Job Summary The Alliance Health Project (AHP) is a community-based LGBTQ+ and HIV-focused clinic located at 1930 Market Street in San Francisco. We provide psychotherapy, substance use counseling, case management, support groups, psychiatry, sexual health services, clinical traineeship, and provider education and training. Our multi-disciplinary staff are a client-centered, collaborative, Harm Reduction-oriented team. We have continued to grow as a community-based clinic by anticipating and addressing the changing needs of our LGBTQ+ and HIV community members for more than 35 years. Our process of continuous improvement of service to our community includes a strong focus on cultural humility and anti-racist service provision. The Clinical Social Worker position is part of the Behavioral Health Services team at the 1930 Market Street Service Center and is responsible for providing case management services to individuals and groups who are living with HIV/AIDS and have substance use disorders and potential co-occurring mental health disorders. This position is part of a larger multidisciplinary behavioral health services team that includes psychiatrists, nurses, social workers, therapists, substance use counselors, case managers and psychologists. This position provides case management services to individuals living with co-occurring diagnoses of substance use issues, mental health issues and medical issues, within a Harm Reduction philosophy. This position works under the clinical supervision of a licensed clinician and in conjunction with other teammates. This position will involve providing case management services consisting mainly of field/home visit work, assessing clients’ service needs and assisting clients with appropriate referrals, including for housing, vocational and other mental health services. Additionally, the Clinical Social Worker will assist clients in creating and meeting treatment plan goals and reduce barriers to care. This position will involve co-location and working closely with Zuckerberg San Francisco General Hospital Positive Health Practice (Ward 86) clinic, working with HIV-positive patients under the Homeless, Aging and Long-Term Survivors (HALT) Center of Excellence model of care. The Alliance Health Project (AHP) provides a wide range of mental health services to persons living with HIV/AIDS, as well as members of the LGBTQ community living with serious mental health challenges. AHP also provides publications and trainings for health care professionals and volunteers. Since AHP’s founding in 1984, AHP has served thousands of clients, counselors and clinicians, justifying its reputation as one of the leading HIV and LGBTQ mental health agencies in San Francisco — a reputation achieved by anticipating and addressing the challenges of the communities AHP serves. Required Qualifications • CSW I: Master’s degree in Social Work, Marriage and Family Therapy or related Counseling field from an accredited school of Social Work, Counseling or similar and knowledge and abilities essential to the successful performance of the duties assigned to the position. • A preliminary job offer and assignment may be extended pending successful background clearance. • CSW II: In addition to the above, possession of the requirements necessary for application for licensure as a LCSW/LMFT/LPCC as determined by the California Board of Behavioral Science Examiners. Preferred Qualifications • One year of experience providing case management work • Experience working with the HIV-positive and LGBTQ+ communities • Bilingual in English and Spanish – written, reading, and oral proficiency. • Bicultural, Experience working collaboratively with Spanish-speaking service providers • Certified, or registered, as Certified Addictions Treatment Counselor (CATC) or other addiction-related certification through the California Association of DUI Treatment Programs (CADTP) or California Consortium of Addiction Programs and Professionals (CCAPP) • Familiar with Avatar, ARIES and Mental Health and Drug Medi-Cal systems of care, • Experience with documenting according to Medi-Cal standards in Avatar, both Mental Health and Drug Medi-Cal systems • Understanding of psychosocial issues facing the LGBTQ and HIV-affected communities. • Experience working within Harm Reduction treatment models, utilizing Motivational Interviewing interventions • Familiarity with evidence-based substance use treatment modalities • Sensitivity to working with cultural, ethnic, and racial issues. • Understanding and experience working with severe mental illness. • Excellent communication and interpersonal skills. • Demonstrated excellence in reliability and attendance. License/Certification CSW I: Within 12 months of employment, successful completion of SF Department of Public Health Involuntary Hold/5150 certification.
