Position Description
In this role you will be responsible for:
- Supporting and expanding the patient-physician partnership
- Assisting in the delivery of continuous and comprehensive primary care within the medical home
- Working with a care team of providers, pharmacists, social workers, behavioral counselors, and nurses
- Initiating the Plan of Care to Address the health maintenance needs of all patients and the chronic care needs of those with one or more the clinically important conditions identified in the practice
- Focusing on population management to include data collection and reporting through the use of clinical registries
- Evaluating data for the purposes of quality improvement, cost savings, utilization and disease management
- Assisting with patient self-management support, transitions in care, and resource connection

- Associates degree in nursing required; BSN preferred
- Current license to practice as a RN in the State of Pennsylvania required
- Minimum 2 years of clinical experience that must include ambulatory physician office experience required
- CPR certification is preferred
- Excellent interpersonal communication and negotiation skills
- Clinical competence in disease management and case management principles preferred
- Clinical competence in applying techniques of assessing psychosocial, behavioral and psychological aspects of a patient’s problem preferred
- Competence in establishing rapport with a patient and community agencies
- Previous experience with data collection and analysis preferred
- Strong organizational, analytical, time management, and computer skills
- Ability to work with peers in team situations, ability to interpret, adapt, and apply guidelines and procedures, demonstrates a learning attitude toward solving problems, and takes initiative to help others required
- Demonstrates the knowledge and skills necessary to provide care for the infant, child, adolescent, adult and senior citizen
Physical Demands:
- Physical Requirements: Sitting, walking, reaching, pulling, bending, stooping, twisting, standing, lifting (up to 10 lbs.). Manual dexterity for operating computer and clinical equipment. Periodic visits to community agencies is required. Requires frequent verbal and written communication in English
- Visual Requirements: 20/20 vision (with/without corrective lenses)
- Hearing Requirements: Ability to hear normal conversations
- Working Conditions: Normal ambulatory office clinical settings with high activity
