Full-Time Case Planner (Outreach)

Job Description

Job Summary:
The Case Planner (Outreach) supports AFG’s mission, and the goal to deliver quality case management services that support youth street and community outreach to at-risk, runaway and homeless teens.
Key Responsibilities:
• Recruit at-risk youth from the community. Conduct street outreach with Peer Educators to locate and inform homeless, runaways and at-risk youth of AFG’s programs and services.
• Provide case management services that help at-risk youth meet basic living needs and find safe/ stable housing, employment, a pathway to education, transportation and parenting support.
• Maintain ongoing contact with participants to provide individual (and family) support services through case planning, crisis intervention, advocacy, mentoring, referrals, resources, etc.
• Develop and facilitate community workshops on topics such as: sex trafficking, teen dating violence, healthy relationships, teen pregnancy, STD prevention, etc.
• Research and develop community resources, make connections, and build partnerships with schools, local organizations and agencies to serve the needs of program participants.
• Attend and speak at community events such as panel discussions and table resource fairs.
• Ensure completion and recording of documentation, and data entry for all project activities.
• Perform other duties, as assigned.
Demonstrated Abilities:
• Solid organizational and case management skills
• Ability to set priorities, take initiative and meet deadlines
• Good communication skills, diplomacy and tact
• Good decision-making, conflict-resolution and effective listening skills
• Ability to handle crisis, cope with job stress and maintain confidentiality
Acquired Knowledge:
• Requires an associate or bachelor’s degree in social work, counseling, psychology, sociology or criminal justice and a minimum of 3-5 years of related experience
• Experience working with youth who are juvenile offenders, homeless or involved in high-risk activities, such as: gangs, violence, drugs, truancy, sexual activity is strongly preferred
• Must possess a current driver’s license, good driving record and reliable transportation
• Requires basic computer software and Internet skills
• Bilingual in English and Spanish preferred
