Wage Range
$21 – $26 Hourly

Position Summary:
The Foundational Community Supports (FCS) Supportive Housing Case Manager/Certified Peer Counselor, under the supervision of the Associate Director of Transitional Health, assists and provides peer support, housing support and case management services by screening, assigning, scheduling and registering clients, making referrals, adding, maintaining and updating client information in the Management Information System; tracking and monitoring existing data for client charts; and maintain document processes with all other Wenatchee Mental Health Services.
Responsibilities:
- Meet with walk-ins and receive phone calls from persons requesting or referring for FCS Permanent Supportive Housing services, screen for urgency and appropriateness of services, make referrals when other services are more appropriate.
- Maintain small housing case management caseload and complete treatment plans, update and review in support of FCS Clients
- Refer cases for intake assessment for other mental health or substance use disorder treatment, counseling, taking into account the clients needs, insurance requirements and program expertise and availability
- Register and maintain client information in the Electronic Medical Record (EMR) and Excel Spreadsheets.
- Work with support, accounting, billing, and clinical staff to assure continuity of client services.
- Monitor and ensure completeness of clinical transactions and reports generated in/by the EMR
- Open and close client files, maintain records in files, monitor and track clinical paperwork to help clinicians ensure timeliness and accuracy.
- Complete appropriate reviews in the EMR client chart
- Provides continuity of care in support of established client/therapist relationships and best practices, coordinates care with providers internally and outside of the agency with a Release of Information
- Is responsible for maintaining safety precautions and constructive, professional relationships with the following team members:
- Medical Services
- Substance Use Services
- Supportive Housing
- Crisis
- Outpatient Services
- Complete appropriate review in the EMR client chart
- Establishes and maintains working relationships with community resources, coordinates consumer treatment with providers and initiates referrals to medical providers and other community resources when appropriate.
- Attend all required individual and group supervision, all staff meetings, clinical trainings, and other trainings, community meetings asked to attend by management or supervisor
- Promote FCS program while meeting with other resources while in the community
- Search for, create report with and secure landlords within the community willing to participate in housing Supportive Housing clients
- Perform other duties as assigned

Physical Requirements:
This position normally requires the physical demands of sitting, standing, walking, bending, lifting or performing other work requiring low physical exertion, talking, hearing and adequate vision on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time.
Non-Physical Requirements:
Education
- Bachelors Degree, preferred
- Associated Degree, GED or High School Diploma, required
Experience
- Mental Health or Substance Use Disorder lived experience required
Special Skills
- Knowledge of Community Mental Health Systems and requirements
- Knowledge of community resources
- Excellent client and consumer communication skills
- Good verbal and written communication skills
- Ability to manage high detail administrative tasks
- Ability to work independently and as a team member
- Bilingual (English/Spanish) preferred, but not required
- Data entry and other computer skills
- Strong interpersonal skills
- Flexibility in scheduled work hours to accommodate evening hours as necessary
- Utilization of Google Calendar
- Utilizing working cell phone
Licensure, Registration, Certification
- Valid Washington State drivers license and minimum required liability insurance for WA State
- Ability to obtain a valid Agency Affiliated Counselor Registration through Washington State Department of Health
- Attendance and completion of Mental Health Certified Peer Counselor Training
Employment is conditional upon:
- Being cleared by criminal background check and fingerprinting when required
