Position Title:  Family Support Specialist: Youth Case Manager

How will I make a difference?

The Family Support Specialist: Youth Case Manager plays a key role in Youth Services’ mission to support youth and families to thrive in their communities. This position is housed within the Youth Programs department and works closely with other youth and young adult case managers, clinical services, and community partners to provide collaborative, comprehensive services.  

The position supports youth ages 12-17 to remain stably housed with their parents/guardians until they are ready to successfully transition to independent living. The Family Support Specialist Youth Case Manager supports youth and to engage with psychoeducation curriculum around goal setting, healthy relationship building, decision making and general mental and physical well being. The position also supports youths’ families with psychoeducation around communication, processing and crisis planning. The case manager connects appropriate youth, ages 12-17 to 21-day stay host homes when the youth/or family needs a break, while they continue to work on stability. The case manager supports the host families throughout their onboarding, training, and hosting responsibilities.

Responsibilities Overview:

Engage youth in individual and family case management:

  • Manage a small caseload of 6-8 youth and families, providing in-home and community-based case management services in collaboration with Youth Services’ clinical department, community partners, schools, child welfare, etc.
  • Provide key services to help youth complete their education, avoid child welfare and juvenile justice involvement; and successfully plan for and/or transition to independent living
  • Refer youth and families to internal and external clinical supports when available

Effective administration:

  • Maintain up to date file documentation per statewide guidelines and medicaid funded requirements

Cultivate intra and inter-agency collaboration:

  • Attend and contribute to regular meetings with community service providers 
  • Participate in building a team culture of collaboration and shared ownership within Youth Programs
  • Support team goals and collaborative problem-solving

Applicants should ideally have the following skills:

  • Experience with collaborative service delivery that values family and youth voice and input in all facets of service
  • Ability to think on the fly with creativity, self-reflection and groundedness
  • Skilled at crisis intervention work; able to respond with intention
  • Commitment to service provision that is centered in a deep-held belief that youth and families fare best when they stay together until the youth is ready to transition to independent living
  • Able to build relationships with young people, families and community partners that are rooted in empathy and mutual accountability
  • Understands that systems-change needs to be rooted in both pragmatism and idealism

How can I be considered for the position?

You can be considered for this position if you meet the following qualifications:

  • This position requires a BA/BS or equivalent amount of education, training, work experience and/or lived experience
  • One year of experience working with adolescents and young adults.
  • This is a field position and frequent travel is required, including client transport. Reliable transportation and comprehensive insurance is required.

This position is full-time at 37.5 hours per week.

Compensation: $20/hr and comprehensive benefits.

Youth Services works in partnership with United Nurses & Allied Professionals, Local 5050, and this position is included in the collective bargaining agreement.
