As a Case Manager, you will be responsible for providing referral and recovery support services to individuals who are struggling with mental health, substance use and behavioral issues. Knowledge, Skills and Abilities and or Competencies required to successfully perform the work:
Strong knowledge of behavioral health practices to provide direct care to client.
Provides clear communication and shares information regularly and concisely.
Demonstrate an ability to utilize continuous improvement techniques and quality research methods to optimize overall program operation and build community relationships.
Ability to coordinate care, delegate tasks, provide referrals and problem solve as needed.
Ability to be clear headed and decisive based on the scope of the position.
Ability to work efficiently and effectively both individually and as part of a professional team.
Ability to appropriately accept feedback through the supervision process – thus displaying the willingness to learn, grow, and improve.
Be proficient in Microsoft Office tools as well as exhibit an ability to learn and utilize electronic health record (EHR) system effectively.
Must have a driving record that is acceptable to the company’s auto insurance policy.
Education, Experience, Licensure, Certification required for entry into position:
Graduation from an accredited college/university with a Bachelor’s degree in human service field. Prior experience in substance use and behavioral health preferred.
