Summary:
Maryland Care Management, Inc. is searching for an enthusiastic Care Coordinator who under direct supervision provides outreach and care coordination services to Maryland Physicians Care MCO (MPC) members The position coordinates members access to care consistent with applicable regulatory and corporate accessibility and availability standards. The position works in conjunction with the UM department and concurrent review to provide continuity of care as members transition to a lower level of care. In addition, the position supports departmental functions such as but not limited to denial letter production and mailing, supporting low birth weight activities, and authorization entry.

Knowledge and Skills:
· Ability to interact and communicate within a team.
· Excellent written and oral skills.
· Strong problem solving, organizational, and time management skills.
· Computer literate, Microsoft Office preferred.
Education and Work Experience:
· Medical Terminology and understanding basic health and medical services.
· Basic Healthcare knowledge including ICD-10 and procedure codes.
· Minimum of two (2) years’ experience with Maryland Medicaid preferred.
