Temporary Butterfly Coordinator – Vera Davis LTC
Job Description
Position Summary:
Reporting under a matrix-management structure, the Butterfly Coordinator will report to the Dementia Advisor and the Director of Care (or Resident Services Supervisor) at the Davis Centre. The role will focus on supporting the ongoing sustainability of emotion-based care approaches at the Davis Centre with the support of the Dementia Advisor, Butterfly Specialist, and leadership team at the home.
Major Responsibilities:
- Use Change Management principles to develop communication and dissemination strategies for the spread and sustainability of emotion-based care approaches.
- Liaise regularly with the Dementia Advisor, Butterfly Specialist, and members of the leadership team to determine sustainability priorities and action plans for the ongoing maintenance of an emotion-based care model.
- Supports the day-to-day operations on a Butterfly home area, including creating the look, sound, and feel of home and facilitating/co-facilitating team huddles to plan and coordinate opportunities for meaningful engagement throughout the day.
- Support Butterfly Specialist and PSN (or designated in-home educator) with in education initiatives, including just-in-time demonstrations to ensure the integrity of the model.
- Complete clinical audits of documentation, including care plans and medication records, for person-centre language and interventions.
- Participates in quality observations and assists with identifying opportunities for growth.
- Support clinical staff in application of person-centred language and intervention approaches for care planning.
- Provide feedback on policy and process revisions, as requested.
- Participate in the analysis and synthesis of information to support policy creation and revision.
- Complete additional work related duties as assigned
Qualifications:
- College diploma or undergraduate degree in health sciences
- Registered with a professional college is an asset
- Minimum of 3 years’ experience in Long Term Care
- Experience working with the Butterfly Model of Care or other emotion-based care philosophies
- Excellent interpersonal skills and conflict resolution skills
- Advanced Computer Skills including MS Office and Electronic Information Management, EHR (Pointclickcare)
- Change Management Knowledge & Skills including training and experience using the Prosci methodology an asset
- Excellent verbal and written communication skills to interact effectively with clients at all levels
- Strong organizational and time management skills
- Experience or involvement in providing simple training and coaching to staff
- Good problem solving skills
- Ability to travel to the various Long Term Care Centres
- Ability to work independently
- Valid “G” licence in good standing
- Experience with clinical auditing of health records would be an asset
Location: The Davis Centre, 80 Allan Drive, Bolton with the flexibility to travel to the other Regionally operated long Term care homes
12 month Contract Full time
Hours of Work: 35 hours per week – 8:30am-4:30pm
Compensation: $71,698 – $89,624 plus comprehensive long term contract benefit package
