POSITION SUMMARY:
Seeking master’s level, full-time, behavioral health case manager. Population of interest is primarily transgender and gender expansive individuals, who may be on probation, living with HIV, and/or experiencing mental health or substance misuse issues.
ESSENTIAL JOB FUNCTIONS:
· Treat all clients with dignity and respect
· Provide mental health evaluations/problem identification and crisis interventions; assess the need for on-going treatment or referral to other agencies
· Provide case management services to participants as needed or assigned
· Help manage the Transgender Center of the Rockies telehealth services
· Collaborate with clients to develop, implement, and revise individualized treatment plans, and monitor treatment progress
· Assist participants with psychological development, social development, and rehabilitation
· Assist participants in accessing community support services and health care
· Provide acuity screenings for clients to assess the impact of HIV in their daily functioning
· Collect grant eligibility documents and maintain current eligibility during treatment episode
· Maintain accurate and timely clinical records consistent with OBH standards in all areas including, but not limited to: counselor admit notes, assessments, daily charting, weekly clinical staffing notes, and discharge summaries
· Build and maintain community partnerships for internal and external referrals
· Participate in team/department meetings, in-services, and supervisory sessions as required
· Collaborate with peer mentors in client treatment goals
· Observe confidentiality requirements
· Perform other duties as assigned
JOB REQUIREMENTS:
EDUCATION:
Master’s Degree in social work or counseling. Working towards licensure as LCSW/LPC and/or CAT/CAS/LAC in the State of Colorado.
EXPERIENCE:
Minimum of one year experience in clinical work and/or case management. Experience working with transgender and gender expansive individuals experiencing mental health and substance use issues and/or individuals involved with the judicial system. Experience working on grants a plus.
SKILLS:
Knowledge of methods of psychotherapy. Knowledge of community resources. Ability to assess crisis situations and intervene appropriately. Familiarity with the DSM V and diagnostic techniques as well as relevant historical knowledge of past DSM’s. Effective written and verbal communication skills. Ability to work flexible and on-call hours, which may be required. Knowledge of LGBTQ+ needs and HIV stigma, education, and treatment as well as understanding cultural responsiveness.
Preferred: Proficiency in Spanish
SUPERVISORY RESPONSIBILITIES: Assist with program supervision of students/volunteers as determined appropriate.
MACHINES AND EQUIPMENT TO BE USED: Computers, calculators, fax machines, telephone, and a variety of other office/clinical equipment.
TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, bending and reaching, ability to use office equipment.
TYPICAL MENTAL DEMANDS: Ability to give, receive, and analyze information; formulate treatment plans; prepare written materials; and articulate goals and action plans. Must demonstrate cultural responsibility and be able to communicate effectively with clients, coworkers, partnering organizations, etc.
OTHER REQUIREMENTS:
· Occasional travel (within Denver Metro)
· Position will primarily work Monday through Friday with some evening and late night work
· Assist other team members in program execution
· Demonstrated ability to constructively work with persons in crisis situations
COMPENSATION:
This is a full-time, hourly position with benefits.
