Position Title: Youth and Young Adult Intake Coordinator

How will I make a difference?
The Youth and Young Adult Intake Coordinator plays a key role in Youth Services’ mission to support youth and families to thrive in their communities. This position is housed within the Youth Programs department and works closely with other youth and young adult case managers, clinical services, and community partners to provide collaborative, comprehensive services.
This position serves as the central intake coordinator for the Youth Programs department, screening and coordinating new intake and referrals from various points of entry and providing brief services. The YYA Intake Coordinator also develops and coordinates skill building and support groups for youth and young adults and their families; cultivates and maintains relationships with community and regional partners; and supports a small caseload of young adult clients as needed.

Responsibilities Overview:
Intake and group coordination:
● Serve as the first point of contact for all people seeking services through referrals or the Youth Services Hotline during business hours
● Screen individuals and families seeking services to determine eligibility, need, risk and identify brief service goals
● Assure all eligible individuals and families have been assessed for Coordinated Entry
● Maintain waitlist of persons seeking services and those who’ve completed intake, as needed
● Coordinate prosocial youth, young adult and/or family skill building groups

Brief support and stabilization services:
● Provide access and referral to basic needs resources
● Provide interim eviction prevention and other short-term housing stabilization
● Support persons receiving services in procuring essential documentation such as identification, homelessness verification, etc.

Intra and inter-agency collaboration:
● Attend and contribute to regular meetings with community service providers
● Participate in building a team culture of collaboration and shared ownership within Youth Programs
● Support team goals and collaborative problem-solving

Additional Information
Applicants should ideally have the following skills:
● Experience with collaborative service delivery that values family and youth voice and input in all facets of service
● Ability to think on the fly with creativity, self-reflection and groundedness
● Skilled at crisis intervention work; able to respond with intention
● Commitment to service provision that is centered in a deep-held belief that youth and families fare best when they stay together until the youth is ready to transition to independent living
● Able to build relationships with young people, families and community partners that are rooted in empathy and mutual accountability
● Understands that systems-change needs to be rooted in both pragmatism and idealism

How can I be considered for the position?
You can be considered for this position if you meet the following qualifications:
● This position requires a BA/BS OR equivalent amount of education, training, work experience and/or lived experience
● One year of experience working with adolescents and young adults.
● This is a field position and frequent travel is required, including client transport. Reliable transportation and comprehensive insurance is required.
