Position Overview
The Division of Developmental Disabilities is seeking to hire a Social Worker II for the Murray Developmental Center located in Centralia, Illinois to perform professional-level social work services as a caseworker in habilitation services and supports to adults with developmental disabilities on an assigned unit. Serves as a professional member of an interdisciplinary team. Serves as liaison with individuals and organizations that are significant to specific individual services. Receives in-service and on-the-job training.
Job Responsibilities
1. Performs professional-level social work services as a caseworker in habilitation services and supports adults with developmental disabilities on an assigned unit.
2. Provides services to individuals served and families assigned in the individual habilitation plan including such activities as individual, group, and family therapy and indicated coordination with community agencies and resources.
3. Completes annual Illinois Client Information System evaluations for assigned individuals.
4. Serves as a professional member of an interdisciplinary team.
5. Obtains guardianship for assigned individuals.
6. Prepares referral placement packets for individuals.
7. Monitors the quality of care and records for assigned individuals served for compliance with applicable standards.
8. Prepares and maintains individual review schedules, individual records, and various statistical reports regarding individuals assigned to the Center.
9. Receives in-service and on-the-job training designed to enhance abilities.
10. Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
1. Requires a master’s degree in social work from a recognized college or university supplemented by one year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled, or other disabled persons.
This class is included as an Upward Mobility Program credential title.
Conditions of Employment
1. Requires ability to prepare complex written and oral reports.
2. Requires the ability to work after business hours, on weekends, and on holidays.
3. Requires the ability to physically restrain individuals as necessary to prevent injury to individuals or others.
4. Requires the ability to pass the IDHS background check.
5. Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
6. Requires ability to meet all agency vaccine/health-related policies and guidance.
