Full-Time Clinical Care Coordinator

Job Summary
The Alliance Health Project (AHP) is a community-based LGBTQ+ and HIV-focused clinic located at 1930 Market Street in San Francisco. We provide psychotherapy, substance use counseling, case management, support groups, psychiatry, sexual health services, clinical traineeship, and provider education and training. Our multi-disciplinary staff are a client-centered, collaborative, Harm Reduction-oriented team. We have continued to grow as a community-based clinic by anticipating and addressing the changing needs of our LGBTQ+ and HIV community members for more than 35 years. Our process of continuous improvement of service to our community includes a strong focus on cultural humility and anti-racist service provision.

The Clinical Care Coordinator position is part of the Behavioral Health Services team at the 1930 Market Street Services Center and is responsible for providing individual and group therapy, care coordination, case management and substance use counseling services to individuals and groups who have severe mental illness and substance use issues and/or are living with HIV/AIDS. This position is part of a larger multidisciplinary behavioral health services team that includes psychiatrists, nurses, social workers, therapists, substance use counselors, case managers and psychologists.

This position provides a continuum of behavioral health services and is intended to be a core component to increasing service access to individuals with co-occurring disorders. All services are provided within a Harm Reduction philosophy, and utilize Motivational Interviewing, Dialectical Behavioral Therapy (DBT) and trauma informed treatment among other interventions. All services are intended to help clients explore their relationship to mental and physical health, as well as their drug(s) of choice. This position works under the clinical supervision of a licensed mental health professional and in conjunction with other teammates. This position involves a combination of triage/screening and referral, crisis assessment and carrying a caseload.

Required Qualifications
CSW I:
• Master’s degree in Social Work, Marriage and Family Therapy or related Counseling field from an accredited school of Social Work, Counseling or similar and knowledge and abilities essential to the successful performance of the duties assigned to the position
• Within 12 months of employment, successful completion of SF Department of Public Health Involuntary Hold/5150 certification.
• One year of experience working within a multidisciplinary team.
• Experience working with persons with HIV/AIDS and with persons identifying as LGBTQ
• A preliminary job offer and assignment may be extended pending successful background clearance.

CSW II: In addition to the above,
• Possession of the requirements necessary for application for licensure as a LCSW/LMFT/LPCC as determined by the California Board of Behavioral Science Examiners.

Note: Physical/health screening required.

Preferred Qualifications
• Certified, or registered, as Certified Addictions Treatment Counselor (CATC) or other addiction-related certification through the California Association of DUI Treatment Programs (CADTP) or California Consortium of Addiction Programs and Professionals (CCAPP)
• Bilingual in English and Spanish – written, reading, and oral proficiency.
• Bicultural, experience working collaboratively with Spanish-speaking service providers
• Familiar with Avatar, ARIES and Mental Health and Drug Medi-Cal systems of care
• Experience with documenting according to Medi-Cal standards in Avatar, both Mental Health and Drug Medi-Cal systems
• Understanding of psychosocial issues facing the LGBTQ and HIV-affected communities.
• Experience working within Harm Reduction treatment models, utilizing Motivational Interviewing interventions
• Familiarity with evidence-based substance use treatment modalities
• Sensitivity to working with cultural, ethnic, and racial issues.
• Understanding and experience working with severe mental illness.
• Excellent communication and interpersonal skills.
• Demonstrated excellence in reliability and attendance.
License/Certification
CSW I:
• Within 12 months of employment, successful completion of SF Department of Public Health Involuntary Hold/5150 certification.

CSW II: In addition to the above:
• Possession of the requirement necessary for application for licensure as a Clinical Social Worker/Marriage and Family Therapist/Professional Clinical Counselor as determined by the California Board of Behavioral Science Examiners.
