JOB DESCRIPTION SUMMARY: The PrEP Peer Support Navigator is a fulltime peer Case Management and Navigation services position with The TransLatin@ Coalition. This position is under the direct supervision of the Vice President & Chief Operations Officer. This position is funded By the California Department of Public Health Center for Infectious Disease, Office of AIDS. The ideal candidate will be working to provide PrEP-Pep navigation services Latinx Trans, Gender Non-Conforming and Intersex individuals (TGI) to provide information and link community members to our PrEParad@X, program. An innovative and comprehensive PrEP/PEP program for (TGI) Latinx individuals in Los Angeles County. The ideal candidate would be proficient in English and Spanish and will have clear professional guidelines about how to successfully work with people of Latinx (TGI) experience and their needs. The ideal candidate will also be knowledgeable about HIV Biomedical prevention strategies and be able to work in collaboration with the social network and outreach coordinator and a cohort of individuals from the TGI Latinx community, community partners and staff members. The ideal candidate must have a vehicle and be available to attend community events to promote programs and services and be able to work evenings and weekends providing outreach where TGI Latinx individuals gather and congregate.

 

ESSENTIAL FUNCTIONS

 

  1. Provide PrEP/PEP resources available in the area for clients in need of services and develop a network with PrEP/PEP providers.
  2. Provide program Intakes and assessments to potential candidates to determine qualification to the PrEParad@x Program.
  3. Provides comprehensive peer case management services and access to biomedical prevention services (PrEP/PEP), ensuring access, follow-up, and adherence to PrEP/PEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance.
  4. Maintains communication between inter, and intra-agency service providers for successful coordination of services.
  5. Provides, and ensures access and active referrals to STIs screening and other supportive services
  6. Collaborates with project staff in advertising and media promotion of the program, including the use of geo-social networking.
  7. Conduct outreach activities with program staff to enlist (TGI) Latinx individuals who are interested in our PrEParad@x program which is an HIV prevention strategy
  8. Actively participate in community events and forums to provide referrals to PrEP services and other supportive services
  9. Ensure the enrollment of clients into PrEP services is successful into clinics who are part of the Center of Excellence for PrEP in Los Angeles County.
  10. Coordinate distribution of incentives to program participants.
  11. Maintain documentation of all client contacts and assist in preparing timely reports on a timely manner
  12. Have the ability to do public speaking at different events including but not limited to: conferences, health fairs, public forums, meetings and community gatherings
  13. Work in collaboration with other existing staff from the TransLatin@ Coalition to be able to do presentations to clients and recruit from internal existing programs
  14. Other activities as required to conduct program and agency objectives.
  15. Other duties can be assigned by Program Director.
  16. This position is based on a grant and can be suspended at any time

 

MINIMUM QUALIFICATIONS- Knowledge, Skills, and Ability

  1. Have a bachelor’s degree in social work, social sciences or at least 2 years of working with people of (TGI) experience.
  2. Proof of employment eligibility
  3. Updated tuberculosis test (annual requirement) – read, reviewed and dated by a clinician 6 months prior to employment
  4. Ability to coordinate multiple projects & tasks simultaneously in a high-pressure environment
  5. Ability to master a wide range of information, taking initiative and adapt in a team environment
  6. Ability to communicate effectively both verbally and in writing
  7. Ability to work with diverse communities (e.g., multi-gender, ethnic, generation, lingual, Lesbian, gay, bisexual, transgender, substance use)
  8. Basic knowledge of Social Determinants, PrEP/PEP, Workforce Development, HIV/AIDS, and related issues
  9. Good problem solving and conflict resolution skills
  10. Great attitude, teamwork and people skills a must
  11. Experience and knowledge of PC hardware and software (Windows, Power Point, Word and Excel a must)

NON-ESSENTIAL QUALIFICATIONS- Knowledge, Skills, and Abilities

 

  1. Relevant community outreach training
  2. Experience and knowledge of MS Access and Outlook Software
  3. Bilingual in Spanish and English Preferred
  4. Knowledge of behavior change interventions
  5. Excellent writing skills
  6. Excellent interpersonal skills

 

SUPERVISORY RESPONSIBILITIES

 

  1. The incumbent in this position does not have formal supervisory responsibilities

 

ENVIRONMENTAL CONDITIONS

 

  1. Ergonomically safe office environment with desktop computer, desk, chair, natural light from windows
  2. Occasional outreach at community festivals, conferences, nightclubs, and other outreach venues that target population frequents. Must be comfortable with environmental conditions at the outreach locations, such as noise level, aggressive patrons, substance use,
