The Role

The Transition Coordinator works directly with transitional age youth to provide career exploration, career readiness and transition support resulting in placement in career training programs, postsecondary education, and/or competitive employment. This position will coordinate program activities and manage relationships with partners in priority industries such as skilled trades, manufacturing, healthcare and others as identified. As part of the transition team, this position will support with JVS’s youth alumni outreach strategy and lead related engagement activities.

Scope

Client Support

  • Through one-on-one and group coaching, support program participants in:
    • Exploring jobs/careers of interest and the associated career pathway training and internship programs
    • Preparing for, and obtaining, competitive employment, and/or entry into career pathway training and internship programs
    • Successful completion of career pathway training and internship programs and successful retention of competitive employment
    • Connecting to programs with caring and appropriate adults to support their continued transition
  • Lead collaboration with employers and career pathway training/educational programs to support participant outcomes
  • Identify participant needs and work with internal and external partners to connect the participant to resources
  • Ensure participants experience a high quality and compassionate experience with JVS

 

Program Coordination

  • Plan and coordinate transition program activities including outreach, planning, participant recruitment, participant enrollment, employer and other guest speakers, curriculum and instruction, access to supportive services and client support as outlined above
  • Propose and implement improvements to increase student support, successful transition, and overall program outcomes
  • Maintain accurate and timely data entry into Salesforce to track client case management activities and grant reporting
  • Cultivate new and maintain existing relationships with community partners that serve as referral, work-based learning, employer and volunteer partners for JVS programs
  • Collaborate with JVS staff at school based programs to outreach to those program participants by showcasing training/employment opportunities and supporting program activities, as appropriate
  • Maintain accurate data on programs, preparing periodic reports to management on program performance and collaborate with coordinators across department and organization

 

Alumni Engagement

  • Support the development of HSB’s alumni communication and engagement strategy
  • Plan and facilitate activities associated with the alumni engagement strategy to foster ongoing connection of alumni to JVS and to connect interested alumni to transition programing and other opportunities as identified

Qualifications

  • Excellent youth engagement and relationship management skills with employer, training, and educational partners
  • Experience supporting youth in learning about jobs/careers and obtaining competitive employment or entry into career training programs
  • Experience facilitating engaging youth workshops both in-person and remotely
  • Experience working with youth and maintaining appropriate boundaries
  • Strong decision making and problem-solving skills
  • Able to utilize an intersectional racial equity lens while working in a multi-cultural environment with diverse staff, partners, and clientele
  • Strong organizational skills, able to manage priorities, meet deliverables and deadlines
  • Able to work independently, as part of a team, and flexible to meet evolving needs of participants, partners, and program
  • Strong computer proficiency including Microsoft Office Suite, and Google Drive
  • Ability to build strong partnerships broadly, with various organizations and with influential stakeholders like community outreach partners and employers
  • Takes initiative, works effectively independently and as part of a team
  • Strong presentation, public speaking and group facilitation skills
  • Strong written communication skills; able to write clearly, concisely, consistently using appropriate tone, and providing context
  • Ability to pass DOJ and FBI background checks in accordance with Education Code Section 45125.1
  • Ability to pass a Tuberculosis Risk Assessment before working in-person with youth participants and every 4 years thereafter in accordance with California Education Code sections 4906 and 87408.6 and Health and Safety Code sections 1597.055 and 121525-121555
  • Has received a full series of vaccination for Covid-19 or, for those who cannot receive the COVID-19 vaccine due to a disability or sincerely held religious believe, are willing and able to engage in weekly COVID-19 testing in accordance with SFUSD policy for Community Based Organizations working with students at school sites
