Established as a community non-profit in 1972, The Center provides targeted programs and services to the full diversity of the San Diego LGBTQ community. The mission of The Center is to enhance and sustain the health and well-being of the lesbian, gay, bisexual, queer, transgender, nonbinary, immigrant, and HIV communities for the betterment of our entire San Diego region. The Center currently has over 75 staff working at five facilities across San Diego County.

POSITION SUMMARY

Under the direct supervision of the Senior Services Program Manager, this position is responsible for the development, implementation, and coordination of a broad array of activities and case management services for LGBTQ and HIV+ seniors who live in the North Park Senior Apartments (NPSA). The case manager will set specific goals with each senior as a part of their caseload, as well as design, and modify case plans, connect seniors to adequate community resources, and track participants’ progress towards program goals. This position will also assist in support of the Senior Services team (located at The Center) in any major events/activities, as needed; as well as serve as a part of the Senior Services team, working together to develop the larger vision for The Center’s Senior Services program. The position is out stationed at the NPSA site. The individual must be a self-starter who provides client-centered and trauma-informed care. Must work collaboratively with partner organizations to create a safe and supportive environment for all seniors. Individuals with lived experience as a member of a historically under-served community (i.e., communities of color, LGBTQ communities, senior community, or immigrant communities, etc.) are strongly encouraged to apply.

 

PRIMARY ROLES & RESPONSIBILITIES:

  • Coordinating senior programs to address HIV testing; voter education; social activities; advocacy; health care education, enrollment, and access; and life skills training at North Park Senior Apartments (NPSA)
  • Providing case management, information, and referrals to seniors, as assigned
  • Complete timely and accurate case notes, data collection, and other documentation for assigned clients into several case management systems and databases
  • Supervising and providing group and workshop facilitation
  • Achieve all contract and/or grant objectives
  • Complete timely program documentation and reporting, ensuring that programs are on track to meet contractual goals and grant deliverables
  • Data entry into Clarity, the regional homeless management information system, as required.
  • Utilize 2-1-1 CIE as a case management tool.
  • Working with property management at NPSA to assist in achieving longevity of housing placement
  • Establish and maintain active working relationships with center program staff, community partners, and other LGBT-affirming providers

REQUIRED QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

  1. Education and/or Experience: A bachelor’s degree in psychology, social work or social services related field is required.
  2. Computer/Office Equipment Skills: Must be comfortable working in PC/Windows-based environments. Must have proficiency with Microsoft Office, particularly Word, Outlook and Excel.
  3. Other Skills: Must demonstrate competence in working with a variety of populations. Must have excellent interpersonal communication skills. Ability to manage multiple tasks with shifting priorities. Ability to supervise, direct and lead staff and volunteers of different cultures.
  4. Language Skills: Fluency in English required.

PREFERRED QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

  1. Education and/or Experience: A minimum of one year of experience in case management is preferred. Prior experience working with LGBTQ seniors is preferred. Related experience can be substituted for the educational requirement on a year-for-year basis.
  2. Other Skills: Lived experience as a member of a historically under-served community (i.e., communities of color, LGBTQ communities, or immigrant communities, etc.) preferred.
  3. Language Skills: Fluency in Spanish is preferred.

 

COMPENSATION & BENEFITS:

  • Compensation: The pay range for this position is $45-48K/annually, 21.63-$23.08/hourly and is commensurate with experience. This is a Full-Time Regular Non-Exempt
